综合护理对儿童骨折的护理效果的meta分析

摘　　要：目的系统评价综合护理用于儿童骨折的护理效果。方法系统检索Cochrane Library、PubMed、EMBase、CBM、CNKI、VIP及万方7个数据库,配合补充检索手段,收集所有综合护理对比常规护理用于儿童骨折康复护理的随机对照试验(randomized controlled trial,RCT)。逐步进行文献筛选和资料提取,评估纳入文献的偏倚风险,应用RevMan 5.3软件进行meta分析。结果共纳入32篇RCT(2 644例患者)。Meta分析结果显示,综合护理组的患儿家长满意度及骨折康复情况均显著高于常规护理组,其差异均有统计学意义(P<0.05),其合并效应量分别为[OR=6.87,95%CI=(4.71,10.01)]和[OR=2.89,95%CI=(2.19,3.83)]。综合护理组的患儿疼痛率、并发症发生率及住院时间均显著低于常规护理组,其差异均有统计学意义(P<0.05),其合并效应量分别为:[OR=0.31,95%CI=(0.17,0.55)]、[OR=0.21,95%CI=(0.15,0.30)]和[MD=-3.25,95%CI=(-4.65,-1.85)]。结论与常规护理模式相比,综合护理用于儿童骨折康复的护理效果更好,能提高患儿家长满意度及骨折康复情况,并降低患儿疼痛率、并发症发生率及住院时间,值得临床护理推广。[Objective]To systematically review the effect of comprehensive nursing intervention（CNI） for children＇s fracture.[Methods]The databases as Cochrane Library,PubMed,EMBase,CBM,CNKI,VIP and Wanfang were searched with computer,and other sources were retrieved as supplying. All relevant randomized controlled trials（RCTs） were collected to assessment the application of CNI for children＇s fracture. After literature screening,data extraction and quality evaluation according to the protocal,meta-analysis were performed using the RevMan 5.3 software.[Results]32 RCTs involving 2644 patients were included.The results of meta-analysis showed that parent satisfaction rate and fracture recovery rate of CNI group were higher than those of control group, and the difference was statistically significant（ P 〈 0. 05）, while the combined effects were [ OR = 6. 87, 95 %CI =（4.71,10.01）] and [OR =2.89,95% CI =（2.19,3.83）] respectively. The pain rate,incidence of complication and length of hospital stay of CNI group were lower than those of control group,and the difference was statistically significant（P〈0.05）,while the combined effects were [OR=0.31,95%CI=（0.17,0.55）],[OR=0.21,95%CI=（0.15,0.30）] and [MD=-3.25,95%CI=（-4.65,-1.85）],respectively.[Conclusion]Compared with routine nursing mode,the application of CNI for children＇s fracture can significantly improve nursing effect,enhance the parent satisfaction rate and fracture recovery rate,and reduce pain rate,incidence of complication and length of hospital stay,which is worthy of clinical promotion.
