葛根黑苏汤联合温针灸对薄性子宫内膜形态学的影响

摘　　要：目的探讨葛根黑苏汤联合温针灸对薄性子宫内膜形态学的影响。方法将90例患者随机分为三组：西药组（A组）、中药＋温针灸组（B组）、中药＋温针灸＋西药组（C组）。观察三组患者排卵日子宫内膜厚度、类型、容积及内膜下血流，随访妊娠率。结果与A组比较，C组子宫内膜厚度、A型内膜及内膜下血流Ⅱ＋Ⅲ型所占的比例明显高于A组（P〈0．05）；B组与A组比较A型内膜及内膜下血流Ⅱ＋Ⅲ型所占的比例高于A组（P〈0．05），但内膜厚度两组比较差异无统计学意义（P〉0．05）；B组与C组子宫内膜厚度比较差异有统计学意义（P〈0．05），三组内膜容积比较C组优于B组，B组优于A组，但三组间比较差异无统计学意义（P〉0．05）；三组临床的妊娠率分别为10％、33.3％、36．7％，B、C两组妊娠率明显高于A组（P〈0.05）。结论葛根黑苏汤联合温针灸能使子宫内膜增厚，A型子宫内膜比例升高，在改善子宫内膜厚度上与戊酸雌二醇有相似的效果，能改善子宫内膜血流状态，有利于胚胎着床，从而提高了妊娠成功率，研究还发现葛根黑苏汤联合温针灸在改善子宫内膜血流状态及提高妊娠率其疗效优于戊酸雌二醇。Objective To study effect of Gegen heimu decoction combined with warm needling to the morphology of thin endometrium. Methods 90 patients were randomly divided into 3 groups ： western medicine group （ group A ） , traditional Chinese medicine ＋ warm needling group （ group B ） , traditional Chinese medicine ＋warm needling＋ western medicine group （ group C ） . The three groups of patients with ovulation days intrauterine membrane thickness, type, volume and blood flow, beneath the follow-up rate of pregnancy were observed. Results Compared with group A, the endometrial thickness, type A type under the endometrium and endometrial blood flow Ⅱ ＋ Ⅲ proportion in group C were significantly higher than group A （ P〈0.05 ） . Compared with group A, the type A type under the endometrium and endometrial blood flow Ⅱ ＋Ⅲ proportion in group B was higher than group A （ P〈0.05 ） , but the lining thickness of the two groups had no statistical significance （ P〉0.05 ） . Compared with group C, endometrial thickness of group B was statistically significant （P〈0.05） , three groups of lining was better than that of group B, group C volume group B was better than that of group A, but the comparison between the three groups had no statistical significance （P〉0.05） . Of three groups of clinical pregnancy rates were 10%, 33.3%, 36.7%, the pregnancy rate of group B, C was obviously higher than that of group A （ P〈0.05 ） . Conclusion The combination of Gegen heisu decoction combined with warm acupuncture can increase the endometrium and increase the proportion of A endometrium. It is similar to estradiol valerate in improving the thickness of the endometrium. It can improve the state of the blood flow of the endometrium, be beneficial to the implantation and improve the success rate of pregnancy.
