神经肌肉刺激治疗仪对压力性尿失禁产妇盆底功能康复的临床研究

摘　　要：目的:探讨神经肌肉刺激治疗仪对压力性尿失禁(SUI)产妇盆底功能康复的影响。方法:选取自然分娩产妇240例,依据随机数字表法将其分成3组,行神经肌肉刺激治疗仪治疗产妇为观察组(80例),行阴道椎体训练为椎体组(80例),行盆底肌训练为对照组(80例);观察3组患者疗效、盆底肌肉功能恢复状况、SUI发生状况和ICIQ-SF量表评分改善情况。结果:观察组总有效率为95.00%,高于椎体组的82.50%和对照组的83.75%,差异有统计学意义(P<0.05);治疗后3组产妇膀胱颈活动度、VSP、WRP及尿道旋转角度均有改善,和治疗前比较差异均有统计学意义(P<0.05),观察组VRP与VSP改善效果比椎体组、对照组好,差异有统计学意义(P<0.05);观察组SUI发生率为2.50%,低于椎体组13.75%和对照组15.00%,差异有统计学意义(P<0.05);治疗后患者ICIQ-SF评分均有改善,观察组效果最佳。结论:自然分娩产妇在分娩后进行神经肌肉刺激治疗仪治疗可使产妇盆底肌功能明显改善,有效预防及降低SUI发生,对产妇康复有积极作用。Objective： To explore the effect of neuromuscular stimulation on functional recovery of female pelvic floor under stress urinary incontinence（ SUI）. Methods： 240 cases of maternal natural childbirth,according to number chart method randomly divided into three groups for neuromuscular stimulation of maternal observation group（ 80 cases）,vaginal vertebral centrum training group（ 80 cases）,underwent pelvic floor muscle training（ as the control group 80 cases）. The pelvic floor muscle function recovery status,SUI status and ICIQ-SF score improvement in the three groups of patients were observed. Results：In the observation group,the total effective rate was 95.00%,higher than that of vertebral group 82.50%and 83.75% in the control group,the difference was statistically significant（ P〈0. 05）. After treatment,the bladder neck mobility,VSP,WRP and urethral rotation angle in three groups were improved as compared to before treatment with the differences statistically significant（ P〈0.05）. VRP and VSP in the observation group was elevated than vertebral body group and control group,with the difference statistically significant（ P〈0.05）. The incidence of SUI of the observation group was 2.50%,lower than the vertebral group 13.75% and control group 15. 00%,the difference was statistically significant（ P〈0. 05）. After treatment,ICIQ-SF scores improved in all patients, with the best effect in the observation group.Conclusion： The natural childbirth parturient in the treatment of neuromuscular stimulation after childbirth can significantly improve the pelvic floor muscle function,effectively prevent and reduce the incidence of SUI,and has a positive effect on maternal rehabilitation.
