血尿酸水平与初诊2型糖尿病患者游离脂肪酸及糖基化终末产物的相关性分析

摘　　要：目的分析血尿酸与初诊2型糖尿病患者的游离脂肪酸及糖基化终末产物之间的相关关系,为糖尿病合并高尿酸血症患者血管病变的保护提供理论基础和预防措施。方法选择110名入院治疗的初诊2型糖尿病患者,同期选择60名健康体检者。计算体重指数BMI,检测空腹血糖和胰岛素、血尿酸、餐后2h血糖和胰岛素、甘油三脂、高密度脂蛋白、低密度脂蛋白、糖化血红蛋白、尿微量白蛋白、游离脂肪酸、糖基化终末产物等指标。结果入院的初诊T2DM患者的血尿酸平均水平316.78±83.38μmmol/L,高于正常人群(286.57±68.32μmmol/L)(P<0.05),血尿酸浓度与BMI、甘油三酯、游离脂肪酸、糖基化终末产物、胰岛素抵抗指数(HOMR-IR)呈正相关(P<0.05或P<0.01),与糖化血红蛋白、高密度脂蛋白和低密度脂蛋白无相关性。结论初诊的2型糖尿病患者血尿酸浓度与BMI、甘油三酯、游离脂肪酸、糖基化终末产物、HOMR-IR密切相关,血尿酸可能是T2DM合并高尿酸血症患者血管病变的独立危险因素,控制尿酸可能有助于预防和改善T2DM病患者的血管病变。Objective To ascertain the correlation of serum uric acid level and free fatty acid,glycation end products,insulin resistance and metabolic index in patients with newly diagnosed type 2 diabetes.Methods 110 patients with newly diagnosed type 2 diabetes mellitus were selected as serum uric acid observation group,while 60 patients without type 2 diabetes mellitus were chosen as control group.Fasting plasma glucose and insulin level,HbA1 C,TG,HDL-C,LDL-C,FFA,AGE and 2-h plasma glucose and insulin level after oral glucose tolerance test were measured.Results The mean±SD values of serum uric acid level in diabetic patients was 316.78±83.38 ummol/L and those for the controls was 286.57±68.32μmmol/L（P〈0.05）.Results of serum uric acid level and BMI、TG、HOMA-IR、FFA、AGE and MA in diabetic patients showed statistically positive correlation（P〈0.05 or〈0.01）.In addition,there was no association between serum uric acid level and HbA1 c or HDL-C（P〉0.05）.Conclusion Uric acid level in patients with newly diagnosed type 2 diabetes is higher than control groups.There is positive correlation between serum uric acid and FFA、AGE and insulin resistance.And uric acid may be involved in the development and progression of diabetic vascular complications.
