中药降糖复方治疗早期糖尿病肾病的临床观察研究

摘　　要：目的:探讨自拟中药降糖复方对早期糖尿病肾病(DN)的改善作用。方法:将60例糖尿病肾病Ⅲ期的患者采用随机数字表法分为对照组(30例)和治疗组(30例)。两组在基础降糖、降压、调脂等常规治疗下,对照组给予厄贝沙坦治疗;治疗组给予厄贝沙坦加服中药降糖复方,两组疗程均为3个月。分别观察两组治疗前后尿微量白蛋白肌酐比值(ACR);评估中医临床证候疗效;检测空腹血糖(FBG)、糖化血红蛋白(Hb Alc)、同型半胱氨酸(Hcy)、超敏C反应蛋白(hs-CRP)指标。结果:与本组治疗前比较,治疗组治疗后ACR的数值,血清中FBG、Hb A1c、Hcy和hs-CRP的含量明显降低(P<0.05,P<0.01)。治疗组ACR有效率70.00%高于对照组40.00%(P<0.05);治疗后Hb A1c和Hcy的降低优于对照组(P<0.05,P<0.01)。治疗组中医临床证候疗效总有效率为66.70%高于对照组的20.00%。结论:此中药降糖复方能有效改善DN患者血糖,减少尿中微量白蛋白的排出,降低血清中Hcy和hs-CRP的含量,延缓肾功能减退进程。Objective： To observe the effects of Zhongyao Jiangtang Decoction on early stage of diabetic nephropathy. Methods： 60 patients in the third stage of diabetic nephropathy were randomly divided into control group（30 cases） and treatment group（30 cases）. Patients in both group were basically treated with dropping of blood sugar and blood pressure,and regulating hyperlipoidemia. After that,patients in the control group were given irbesartan,while patients in the treatment group were given irbesartan and Zhongyao Jiangtang Decoction,both treatment lasted for 3 months. Before and after the therapy,the ratio of urinary albumin and creatinine（ACR）,clinical symptoms of traditional Chinese medicine（TCM）,fasting blood glucose（FBG）,glycosylated hemoglobin（Hb Alc）,homocysteine（HCY） and ultra sensitive C reactive protein（hs-CRP） were detected to judge the clinical effects of the treatments. Results： The concentration of ACR,FBG,Hb A1 c,HCY and hs-CRP were lower after the treatment（P〈0. 05,P〈0. 01）. The effective rate of ACR is higher than control group（P〈0. 05）,with 70. 00% and 40. 00% respectively. And the decrease of Hb Alc and HCY in treatment group is more obvious than control group（P〈0. 05,P〈0. 01）. Also,the effective rate of clinical symptoms of traditional Chinese medicine（TCM） in treatment group is 66. 70%,higher than control,which is 20. 00%. Conclusion： Zhongyao Jiangtang Decoction can effectively ameliorate blood glucose,reduce ACR,HCY and hs-CRP in serum,so as to delay the process of renal dysfunction.
