加味散瘀生精汤对精索静脉曲张不育症腹腔镜精索静脉高位结扎术后精液质量及血清FSH、LH水平的影响

摘　　要：目的:探究加味散瘀生精汤联合腹腔镜精索静脉高位结扎术对精索静脉曲张不育症患者术后精液质量及血清黄体生成素(Luteinizinghormone,LH)、卵泡刺激素(Follicle-stimulatinghormone,FSH)水平的影响。方法:选取符合标准的精索静脉曲张不育症患者68例,随机分为对照组和试验组,每组各34例。对照组行腹腔镜精索静脉高位结扎术治疗,试验组行腹腔镜精索静脉高位结扎术+加味散瘀生精汤治疗。统计2组治疗效果,并对比治疗前后2组精液质量[精子形态、精液密度及精子活力(a+b)]及血清LH、FSH水平。结果:相较于对照组,治疗后试验组精子活性、精液密度均较高,精子畸形率及血清FSH、LH水平均较低,差异有统计学意义(P<0.05);试验组治疗总有效率97.06%(33/34)高于对照组76.47%(26/34),差异有统计学意义(P<0.05)。结论:对精索静脉曲张不育症患者联合采用腹腔镜精索静脉高位结扎术、加味散瘀生精汤治疗,可有效改善患者精液质量,明显降低血清FSH、LH水平,效果较为显著。Objective： To investigate the effect of Jiawei Sanyu Shengjing decoction combined with laparoscopic high ligation of spermatic vein on the postoperative quality of semen and the level of serum luteinizing hormone（LH） and follicle-stimulating hormone（FSH） in patients with varicocele infertility. Methods： A total of 68 cases of varicocele infertility in our hospital from September 2015 to February 2017 were selected and grouped according to the treatment plan, 34 cases in each group. The control group was treated with laparoscopic high ligation of spermatic vein, and the experimental group was treated with laparoscopic high ligation of spermatic vein＋Jiawei Sanyu Shengjing decoction. The curative effects of two groups were recorded. The quality of semen [sperm morphology, sperm density and sperm motility（a＋b）] and the level of serum LH and FSH of two groups before and after treatment were compared. Results：Compared with the control group, the sperm activity and semen density were higher in the experimental group, and the sperm malformation rate and the serum FSH and LH levels were lower（P〈O.05）. The total effective rate of treatment in the experimental group was 97.06%（33/34）, higher than that of the control group 76.47%（26/34）（P〈0.05）. Conclusion： Jiawei Sanyu Shengjing decoction combined with laparoscopic high ligation of spermatic vein in patients with varicocele infertility can effectively improve the quality of semen and significantly reduce the level of serum FSH and LH, and the effect is more significant.
