慢性牙周炎患者口腔胃幽门螺杆菌感染的相关性

摘　　要：目的研究慢性牙周炎患者口腔胃幽门螺杆菌(Hp)感染的相关性,为临床诊疗提供依据。方法选取2016年1月-2017年3月医院收治的慢性牙周炎患者70例为研究组,另选取同期牙周健康者70例为对照组,应用特异性尿素酶C、cageA基因设置引物,以聚合酶链式反应(PCR)法检测尿素酶C基因和cageA基因并计算含漱液与牙菌斑中Hp感染阳性率。结果研究组含漱液尿素酶C基因阳性率、尿素酶C基因和cageA基因阳性率分别为68.57%(48/70)、31.43%(22/70)高于对照组(P<0.05),试验组与对照组患者的菌斑尿素酶C基因阳性率、尿素酶C基因和cageA基因龈下阳性率分别为65.22%和20.83%高于龈上(P<0.05),研究组龈上尿素酶C基因阳性率为33.85%高于对照组(P<0.001),而龈下尿素酶C基因和cageA基因阳性率为14.91%高于对照组(P=0.015),两组龈上尿素酶C基因和cageA基因阳性率差异无统计学意义;中、重度牙周炎患者Hp检出阳性率分别为75.00%、83.33%均高于轻度患者(P<0.05)。结论口腔Hp感染与慢性牙周炎发病、发展具有一定相关性。应保持良好的口腔卫生,降低口腔Hp感染,改善牙周炎症状。OBJECTIVE To investigate the correlation between oral Helicobacter pylori（Hp）infection in patients with chronic periodontitis,so as to provide the basis for clinical diagnosis and treatment.METHODS A total of 70 patients with chronic periodontitis treated in our hospital from Jan.2016 to Mar.2017 were selected as the study group.Another 70 healthy persons with periodontal disease were selected as the control group.Specific urease C and cage A genes were used to set primers,the specific urease C and cage A genes were detected by polymerase chain reaction（PCR）,and the positive rates of Hp infection in gargle and plaque were calculated.RESULTS The positive rate of gargle urease C gene and cageA gene of the study group were 68.57%（48/70）and 31.43%（22/70）,which were was higher than those of the control group（P〈0.05）.The subgingival positive rates of urease C gene and cageA gene in the two groups were 65.22% and 20.83%,which was higher than supergingival positive rates,and the difference was significant（P〈0.05）.The supergingival urease C gene positive rate of the study group was 33.85%,which was significantly higher than that of the control group（P〈0.001）,and subgingival positive rates of urease C gene and cageA gene in the study group（14.91%）was significantly higher than that in the control group（P〈0.05）.The positive rates of the supergingival urease C gene and the cageA gene between the two groups were not significant.The Hp positive rates of patients with moderate and severe periodontitis were 75.00%and 83.33%,which was significantly higher than that with light periodontitis（P〈0.05）.CONCLUSIONOral Hp infection is associated with the onset and development of chronic periodontitis.We should maintain good oral hygiene,reduce oral Hp infection and improve periodontitis symptoms.
