灭荨汤联合抗组胺药物治疗慢性荨麻疹疗效及安全性评价

摘　　要：目的：观察灭荨汤联合抗组胺药物治疗慢性荨麻疹的有效性以及安全性,分析灭荨汤联合抗组胺药物在慢性荨麻疹治疗中的临床价值。方法：将医院在2014年5月—2016年6月治疗的300例慢性荨麻疹患者根据随机数字表法均分成两组,即灭荨汤组（灭荨汤＋枸地氯雷他定片＋盐酸西替利嗪片治疗）和对照组（枸地氯雷他定片＋盐酸西替利嗪片治疗）,每组各150例患者,在治疗前后观察慢性荨麻疹症状积分变化,并于治疗2疗程后客观评价各组治疗效果、不良反应和复发率。结果：治疗后,灭荨汤组风团评分（0.59±0.70）分、瘙痒评分（0.75±0.66）分、总积分（1.35±1.24）分、复发率（1个月13.33%、3个月18.67%、6个月26.00%）显著低于对照组,临床有效率（94.00%）显著高于对照组（67.33%）,灭荨汤组湿热内蕴证疗效（81.82%）显著低于风寒束表证（97.50%）、风热袭肺证（96.30%）、血虚风燥证（94.12%）,P〈0.05,差异具有统计学意义;两组均未出现严重不良反应。结论：灭荨汤联合抗组胺药物对慢性荨麻疹转归有显著改善作用,其不仅促进临床症状改善,而且有效降低复发风险,是慢性荨麻疹安全、高效的较为理想治疗方案。Objective：To observe the effect and safety of Mieqian Decoction combined with antihistamines in the treat- ment of chronic urticaria and analyze the clinical value of Mieqian Decoction combined with antihistamines in the treat- ment of chronic urticaria. Methods ： A total of 300 cases of chronic urticaria patients in our hospital in June 2015 - May 2016,according to the random number table method, were divided into two groups, namely the Miqian Decoction （ Mieqian Decoction ＋ Desloratadine Citrate Disodium Tablets ＋ cetirizine hydrochloride ） and control group （ Deslorata- dine Citrate Disodium Tablets ＋ cetirizine hydrochloride） with 150 cases in each group. Before and after treatment, we observed the symptoms, and evaluated adverse reactions and recurrence rate after 2 courses of treatment. Results ： After treatment, Mieqian Decoction group＇ s wheal score （ 0.59 ＋ 0.70 ） points, pruritus score （ 0.75 ＋ 0.66 ） points, total score （ 1.35 ＋- 1.24） points, the recurrence rates （ 13.33% of l month, 18.67% of 3 months, 26% of 6 months） were significantly lower than those of the control group. The clinical efficiency （94%） was significantly higher than that of the control group （67. 33% ）. Mieqian Decoction group＇s effective rate of endoretention of damp -heat syndrome （81.82% ） was significantly lower than that of cold - wind attacking the exterior syndrome （97.50%） , wind - heat attac- king the lung syndrome （96.30%）, and blood deficiency and wind dryness syndrome（94.12% ）, and the difference was statistically significant（ P 〈0. 05 ）. The two groups had no severe adverse reactions. Conclusion： Mieqian Decoction com- bined with antihistamines has significant effect on improving the prognosis of chronic urticaria, which not only promotes the improvement of clinical symptoms, but also reduces the risk of recurrence. It is a safe and efficient ideal treatment for chronic urticaria.
