闭合复位或小切口撬拨复位外固定支架结合克氏针固定微创治疗桡骨远端C3型骨折效果观察

摘　　要：目的为了提高桡骨远端C3型骨折的治疗效果,分析采用闭合复位或小切口撬拨复位外固定支架结合克氏针固定微创治疗的价值。方法 60例桡骨远端C3型骨折患者,根据治疗方案的不同分为对照组和治疗组,每组30例。对照组患者接受切开复位钢板螺钉内固定治疗,治疗组患者接受闭合复位或者小切口撬拨复位外固定支架结合克氏针固定微创治疗。比较两组患者手术情况及桡骨远端骨折复位情况。结果治疗组手术时间、术中术后失血量、骨折愈合时间及住院时间均少于对照组,差异具有统计学意义(t=10.89、21.03、2.52、10.35,P<0.05)。对照组尺偏角、桡骨高度均大于治疗组,掌倾角小于治疗组,差异具有统计学意义(t=2.69、5.09、2.62,P<0.05)。结论桡骨远端C3型骨折患者接受闭合复位或者小切口撬拨复位外固定支架结合克氏针固定微创治疗,不仅具有创伤小、术后易恢复的优点,而且术后对腕关节活动影响小,更容易被患者所接受,值得推广。Objective To analyze the value of closed reduetion or small incision prying reduetion and external fixator combined with Kirschner wire fixation for minimally invasive treatment, so as to improve the treatment effect of distal radius C3 fracture. Methods A total of 60 type C3 distal radius fractures patients were divided by different treatment regimens into control group and treatment group, with 30 cases in each group. The control group received open reduction and internal fixation with plate and screw, and the treatment group received closed reduction or small incision prying reduction and external fixator combined with Kirschner wire fixation for minimally invasive treatment. The operation condition and the reduction of distal radius fractures of the two groups were compared. Results The treatment group had less operation time, intraoperative and postoperative bleeding volume, fracture healing time and hospitalization time than the control group, and their difference was statistically significant（t=10.89, 21.03, 2.52, 10.35, P〈0.05）. The control group had larger ulnar angle, sacral height than the treatment group, and smaller palmar angle than the treatment group. Their difference was statistically significant（t=2.69, 5.09, 2.62, P〈0.05）. Conclusion For patients with type C3 distal radius fractures, closed reduction or small incision prying reduction and external fixator combined with Kirschner wire fixation not only have the advantages of small trauma and easy recovery after operation, but also has little effect on wrist joint activity after operation, so it is easier to be accepted by the patients. It is worth popularizing.
