显微镜下精索静脉结扎术联合高压氧治疗精索静脉曲张伴精液异常的疗效观察

摘　　要：目的 探讨显微镜下精索静脉结扎术联合高压氧治疗精索静脉曲张（varicocele,VC）伴精液异常的疗效及高压氧治疗的作用机理.方法 96例VC患者,随机等分为对照组及实验组,对照组单纯行外环下显微镜外科精索静脉结扎术,实验组行外环下显微镜外科精索静脉结扎术联合高压氧（hyperbaric oxygenation,HBO）,观察两组治疗前后精液质量改善情况,并随访1年,观察两组患者配偶受孕情况.结果 两组术后精液质量均较术前改善（P〈0.05）,但实验组精液质量改善情况明显优于对照组（P〈0.05）,且实验组患者配偶受孕率高于对照组.结论 显微外科精索静脉结扎术配合HBO在治疗VC伴精液异常的患者方面,与单纯行手术治疗相比,不仅可以显著改善精液质量,而且可增加患者配偶受孕率,可作为治疗VC的有效手段,值得临床推广.Objective To investigate the effect of microscopic surgical ligation of spermatic vein combined with hyperbaric oxygen in the treatment of varicocele （VC） with abnormal semen and the mechanism of hyperbaric oxygen therapy.Methods A total of 96 patients with VC,they were randomly divided into control group and experimental group,the control group underwent microscopic surgical ligation of the spermatic vein under external ring,the experimental group underwent microscopic surgical ligation of spermatic vein under external ring with hyperbaric oxygenation（HBO）,and the improvement of semen quality before and after treatment was observed in two groups,followed up for 1 years,and the pregnancy rate of the spouses of the two groups were observed.Results The semen quality of the two groups was better than that before the operation,the improvement of semen quality in the experimental group was significantly better than that in the control group,and the pregnancy rate in the experimental group was higher than that in the control group.Conclusions Microsurgical spermatic vein ligation combined with HBO in the treatment of VC patients with abnormal semen,compared with simple surgery,not only can significantly improve the quality of semen,but also increase the rate of pregnancy,which can be used as an effective means of treatment of VC and be worth to be spread.
