左房容积指数对H型高血压患者左室舒张功能的评价

摘　　要：目的应用左房容积指数(LAVI)评价H型高血压患者的左室舒张功能,并探讨与其他舒张功能指标的相关性及可能的临床价值。方法选取原发性高血压患者159例,依据血同型半胱氨酸(HCY)水平将其分为HCY<10μmol/L组即非H型高血压组(A组)54例和HCY≥10μmol/L组即H型高血压组(B组)105例。另选取24例血压正常者作为对照组(N组)。实时三维全容积法测量左房容积(LAV),并根据公式LAVI=LAV/BSA计算LAVI。结果 A、B组LAVI较N组明显增高(P<0.05),且B组LAVI明显高于A组(P<0.05)。A、B组LAVI与E/Em呈显著正相关(P<0.05);与Em/Am及E/A呈显著负相关(P<0.05)。A、B组HCY与Em/Am呈负相关(P<0.05);与收缩压、E/Em、LAVI呈正相关(P<0.05),其中与E/Em、LAVI的相关系数较高,分别为0.839、0.666;HCY与高血压病程、年龄、舒张压水平、LVEF及E/A无明显相关(P>0.05)。结论 LAVI与传统的超声指标存在良好相关,可更广泛的应用于临床评价左室舒张功能。HCY与多项舒张功能指标显著相关,尤其是与E/Em,提示高血压患者左室舒张功能改变可能与HHCY有一定关系。Objective To evaluate left ventricular diastolic function in patients with H types of hypertension by left atrial volume index（LAVI）and to carry on the preliminary discussion of its possible clinical significance.Methods One hundred and fifty-nine patients with essential hypertension were enrolled in the research.And they were divided into group A（hypertension with HCY10μmmol/L）and group B（hypertension with HCY≥10μmol/L）Twentyfour healthy volunteers served as normal control subjects（group N）.The left atrial volume were measured by realtime three-dimensional echocardiography（RT-3 DE）,then left atrial volume index（LAVI）was calculated.ResultsCompared with group N,the LAVI were higher in group A and group B（P〈0.05）.In the patients with hypertension there was a positively correlation between HCY and systolic blood pressure,LAVI or E/Em（P〈0.05）and there was a negatively correlation between HCY and Em/Am（P〈0.05）.In which the correlation coefficient of HCY with the E/Em and LAVI was higher,respectively 0.839,0.666.Conclusions LAVI had good correlation with the traditional ultrasound indexes and my be more widely used to evaluate of left ventricular diastolic function.HCY had significant correlation with many diastolic function parameters,especially E/Em,suggesting that HHCY my be some relationship with left ventricular diastolic dysfunction.
