摘 要:目的评价利用实验室在线质控数据对实验室血常规检测项目不确定度进行评定的适用性。方法利用血常规在线质控和卫生部室间质评数据,根据Nordtest准则分别得出红细胞计数(RBC),血红蛋白(HGB),红细胞压积(HCT),平均红细胞体积(MCV),平均血红蛋白含量(MCH),平均血红蛋白浓度(MCHC),血小板计数(PLT),白细胞计数(WBC)的合成不确定度及扩展不确定度。结果在以上项目中,在线质控的PLT和WBC扩展不确定度分别为U=12.30和U=9.27,略高于室间质评的PLT和WBC扩展不确定度U=11.64和U=7.71,其余各项在线质控的扩展不确定度均低于室间质评的扩展不确定度。但各项的扩展不确定度均包含于中华人民共和国卫生行业标准提出的临床血液学检验常规项目分析质量要求的允许总误差。结论利用实验室在线质控数据对实验室血常规检测项目不确定度进行评定是可行的,且比利用室问质评进行不确定度评估更方便、更可靠。Objective To evaluate the applicability of laboratory on-line quality control data for assessing the uncertainty of laboratory blood routine examination. Methods We made use of blood routine on-line quality control and room quality evaluation data of Ministry of health. According to the nordtest criteria we obtained the red blood cell (RBC), hemoglobin (HGB), hematocrit (HCT), mean corpuscular volume (MCV), mean corpuscular hemoglobin (MCH), mean corpuscular hemoglobin concentration (MCHC), platelet (PLT), white blood cell (WBC) with uncertainty and expansion uncertainty. Results In the above project, the online quality control of PLT and WBC expanded uncertainty were U=12.30 and U=9.27, greater than the external quality assessment of PLT and WBC expanded uncertainty U=11.64 and U=7.71. The rest of the online quality control of uncertainty is less than the expansion of quality assessment of uncertainty. However, the expansion uncertainty is included in the allowable total error of the analytical quality specifications for routine tests in clinical hematology of the Health Industry Standards of People's Republic of China. Conclusion It is feasible to evaluate the uncertainty of laboratory blood routine examination by using laboratory on-line quality control data, and it is more convenient and more reliable than uncertainty evaluation with external quality assessment.
