肝郁气滞型围绝经期非器质性失眠与其他类型睡眠障碍兼夹证素的特点研究

摘　　要：目的探讨肝郁气滞型围绝经期非器质性失眠及其他常见睡眠障碍类型(如睡眠-呼吸暂停、周期性腿动、夜惊、梦魇)的证素兼夹特点,分析围绝经期睡眠障碍的中医病机特点。方法收集符合肝郁气滞型围绝经期睡眠障碍病例,共269例。其中,非器质性失眠病例共139例,其他睡眠障碍类型共130例,采用中医四诊信息采集表采集病史,制定四诊信息采集量表,分析比较两组兼夹病位、病性证素的特点。结果非器质性失眠兼夹病位证素以肾、心、心神为主,兼夹病性证素以阴虚、血虚、气虚、阳虚为主;其他类型睡眠障碍兼夹病位证素以肾、心、胆为主,兼夹病性证素以气虚、阴虚、血虚、阳虚为主。结论肝郁气滞型围绝经期妇女非器质性失眠与其他常见睡眠障碍的患者,同时兼夹不同病位、病性证素,临床上分别针对不同证素兼夹特点加以对应治疗,以提高临床疗效。Objective：To observe the regularities of concurrent syndrome elements on liver depression type of perimenopausal women with non-organic insomnia and other common sleep disorders, and investigate the pathogenesis of Traditional Chinese Medicine on perimenopausal women sleep disorders. Method：According to Zhu Wenfeng＇s syndrome differentiation principle, a total of 241 women with perimenopausal sleep disorders were calculated and divided into different grades, at the same time record every concurrent syndrome element, and finally the above data were statistically analyzed. Results： The syndrome of non-organic insomnia is mainly kidney, heart and heart-mind, while the main disease syndrome is Yin deficiency, blood deficiency, Qi deficiency and Yang deficiency. The other common sleep disorder group syndrome are mainly kidney, heart and bile, while the syndrome of disease is mainly with Qi deficiency, Yin deficiency, blood asthenia and Yang deficiency. Conclusion ：The degree of the liver depression in perimenopausal women with non-organic insomnia was higher than that of other common sleep disorders. The Traditional Chinese Medicine pathogenesis of other common sleep disorders may more complicated than non-organic insomnia.
