摘 要:目的探讨2型糖尿病(T2DM)患者脂溶性维生素A、D、E的变化及与其血脂的相关性。方法纳入74例T2DM患者作为病例组,选取同期健康体检者59例为对照组,采用液相色谱一串联质量光谱法技术检测两组研究对象的血清脂溶性维生素A、D、E水平并进行比较,分析T2DM患者血清脂溶性维生素水平变化及其与血脂的相关性。结果与对照组比较,病例组患者维生素A、D、E水平明显降低,差异均有统计学意义(P〈0.05)。血清维生素A、D、E缺乏诊断T2DM的受试者工作特征(ROC)曲线下面积分别为0.643、0.667和0.732(P〈0.01),且维生素A、E均与糖化血红蛋白(HbAlc)、甘油三脂(TG)、低密度脂蛋白胆固醇(LDL—C)呈正相关,与高密度脂蛋白胆固醇(HDL—C)呈负相关(P〈0.05);调整季节因素后,维生素D与上述两种脂溶性维生素表现出一致的相关性。结论T2DM患者血清脂溶性维生素水平明显降低,维生素A、D、E缺乏对T2DM的辅助诊断具有一定意义,并且与HbA1c、TG、LDL-C、HDL—C均有相关性。Objective To explore the changes of serum fat-soluble vitamins A, D, E in patients with type 2 diabetes mellitus(T2DM) and their correlations between the serum fat-soluble vitamins with blood lipids. Methods Seventy-four patients with T2DM were included into case group and contemporaneous 59 persons undergoing physical examination were included into the control group. The levels of vitamin A, D and E were measured by liquid chromatography-tandem mass spectrometry and compared. Changes of serum fat-soluble vitamins and their correlations with blood lipids were analyzed. Results The levels of vitamin A, D and E in case group were significantly lower compared with those in control group and the difference were statistically significant( P 〈 0.05 ). The areas under the receiver operating characteristic (ROC) curve of the subjects with serum vitamin A, D and E deficiency were 0. 643,0. 667 and 0. 732 ( P 〈 0. 01 ), respectively. Vitamin A and E were positively correlated with glycosylated hemoglobin (HbAlc) ,triglyeeride(TG) and low-density lipoprotein cholesterol (LDL-C) while negatively correlated with high-density lipoproteincholesterol ( HDL-C, P 〈 0.05 ). After the adjustment of seasonal factors, vitamin D showed a consistent correlation with vitamin A and E. Conclusion Levels of serum fat-soluble vitamins in patients with T2DM significantly reduce. The deficiencies of serum fat-soluble vitamins A, D, E make a certain sense on the auxiliary diagnosis of T2DM. There are correlations between serum fat-soluble vitamins with HbA1 c,TG, LDL-C and HDL-C.
