摘  要:研究2015年山东省烟台地区手足口病的病原谱,以及柯萨奇病毒A组4型(Coxsackievirus A4,CV-A4)的分子流行病学。采用实时荧光逆转录聚合酶链反应方法,对2015年烟台地区采集于手足口病患者的696份样品进行总肠道病毒(EV)、柯萨奇病毒A16(CV-A16)型和肠道病毒A71型(EV-A71)的核酸检测,并对非EV-A71非CV-A16的EV阳性标本,进行CV-A4的核酸检测。对CV-A4阳性标本进行病毒分离,对病毒分离阳性的CV-A4标本进行全长VP1区扩增,并进行核苷酸序列测定和分析,然后进行核苷酸/氨基酸相似性分析和系统发育分析。从696份标本中检出肠道病毒阳性标本412份,其中101份为CV-A16,76份为EV-A71,16份为CV-A4。9份CV-A4阳性标本成功病毒分离,并进行了全长VP1区核苷酸序列分析,结果表明,其核苷酸相似性为91.04%~100%,氨基酸相似性为95.24%~100%。系统进化分析显示9株CV-A4烟台株均属于G1基因群。与参考株CVA4-High Point-AY421762-genome相比,氨基酸序列的第20、34,102、111、143、184、185、200及304位氨基酸出现突变,本研究的9株分离株的这些位点也有不同,表明烟台地区的CV-A4有其独特的突变位点。CV-A4是引起2015年烟台地区手足口病发病的病原体之一,本次分离到的CV-A4均属于G1基因群进化分支。We wished to undertake etiologic surveillance of hand,foot and mouth disease(HFMD)and to analyze the molecular epidemiologic characteristics of coxsackievirus A4(CV-A4).Real-time fluorescence reverse transcription-polymerase chain reaction of 696 HFMD samples in Yantai in 2015 revealed enterovirus(EV),coxsackievirus A16(CV-A16)and intestinal virus A71(EV-A71).Virus separation,determination of nucleotide/amino-acid sequences and phylogenetic analyses were done.We found 412 EVs from 696 specimens,101 of which were CV-A16,76 were EV-A71 and 16 were CV-A4.Nine CV-A4 positive specimens were isolated,and nucleotide-sequence analyses of the whole VP1 region were conducted.The nucleotide similarity was 91.04%-100%,and amino-acid similarity was 95.24%-100%.Compared with reference strains,20,34,102,111,143,184,185,200,304 amino-acid mutations were different.CV-A4 was found to be a branch of the G1 gene group.
