摘 要:目的检测Toll样受体(TLR)2在耐多药肺结核患者外周血中性粒细胞(PMN)的水平,探讨其在结核分枝杆菌耐药机制中的意义。方法在药物干预前后,应用酶联免疫吸附试验(ELISA)技术,分别检测26例耐多药肺结核患者、30例非耐药肺结核患者和30例正常对照者外周血PMN培养上清液中TLR2的水平。结果干预前耐药组肺结核(PTB)患者外周血PMN培养上清液中TLR2的平均浓度为(11.789 6±3.346 0)ng/mL,低于非耐药组PTB患者外周血PMN培养上清液中TLR2的平均浓度(17.878 0±1.326 4)ng/mL(P=0.000)。同时,耐药组TLR2水平也低于正常对照组(14.719 7±2.635 4)ng/mL(P=0.001)。干预前后,耐药组TLR2水平差异无统计学意义,而非耐药组患者外周血PMN上TLR2的水平与干预前相比有降低(P=0.000)。结论 TLR2可能在耐多药结核分枝杆菌对宿主产生免疫逃避上发挥了关键作用,对于其具体作用机制,仍需进一步研究。Objective To detect the levels of TLR2 on peripheral blood polymorphonuclear cell (PMN) in patients with multidrug-resistant pulmonary tuberculosis (MDR-PTB) and explore their significance in the drug resistance mechanism of Mycobacterium tuberculosis. Methods Totally 56 patients with MTB were divided into two groups: MDR-MTB group (n=26) and non-MDR-MTB group (n=30) .Meanwhile, 30 healthy volunteers were assigned as control group.Followed by any treatments, ELISA assays were employed to detect TLR2 levels in the supernatant of peripheral blood PMN. After systemic treatments , TLR2 levels in MTB patients of two groups were detected again. Results Before treatment,the TLR2 average level of PMN culture supernatant in MDR-MTB group was (11.789 6±3.346 0) ng/mL, which was significantly lower than non-MDR-MTB group (17.878 0±1.326 4) ng/mL and control group (14.719 7±2.635 4)ng/mL,(P=0.000, P=0.001,respectively). There was no statistic difference of TLR2 in MDR-MTB group pre- and post- the treatment.However, the TLR2 levels decreased significantly in non-MDR-MTB after treatment,compered with pre-treatment(P=0.000). Conclusion These finding indicated that TLR2 may play the critical roles in immune evasion of MDR-MTB. However, more studies should be carried out to further evaluation.
