血管回声跟踪技术评价绝经后骨质疏松症患者颈动脉弹性功能

摘　　要：目的：探讨血管回声跟踪（Echo tracking，ET）技术评价绝经后骨质疏松症（osteoporosis，OP）患者颈动脉弹性的应用价值。方法：选取52例绝经后OP患者（OP组）及48例正常绝经后女性（对照组），应用ET技术检测颈动脉弹性，主要检测参数包括：僵硬度（β）、增大指数（AI）、顺应性（AC）、脉搏波传导速度（PWVβ）、压力-应变弹性系数（Ep）；后将骨密度T值与颈动脉弹性参数β、AI、AC、PWVβ、Ep进行相关性分析。结果：OP组β、PWVβ、Ep高于对照组（P〈0.01）、AC低于对照组（P〈0.05），两组间AI比较差异无统计学意义（P〉0.05）；T值与β、PWVβ、Ep呈负相关、与AC呈正相关、与AI无相关性。结论：自然绝经后1-10年内，OP患者颈动脉弹性较正常绝经后女性减低，且颈动脉弹性与骨密度相关;ET技术能早期、定量评估绝经后OP患者颈动脉弹性功能变化。Objective To explore the application value of E-Tracking （ET） technique in assessment of carotid elasticity in postmenopausal women with OP.Methods：Fifty-two cases with osteoporosis （OP group） and forty-eight normal cases （control group） were chosen.The elasticity parameters including stiffness index（β）,augment index（AI）,compliance （AC）,pulse wave velocity （PWVβ）,pressure strain elastic modulus（Ep） were measured with ET technique.Then the correlation between T-score and elasticity parameters of ET were analyzed.Results：β,PWVβ,Ep of carotid artery in OP group were higher compared with the control group （P〈0.01）,AC was decreased compared with the control group （P〈0.05） ,and there was no significant difference in AI between two groups.T-score had negative correlation with β,PWVβ,Ep,and positive correlation with AC,and no correlation with AI.Conclusion：The elasticity function of common carotid artery of postmenopausal women with OP were decreased compared with healthy postmenopausal women within one to ten years after menopause,and the carotid elasticity was correlated with bone minerals density （BMD）;ET technique can assess the elasticity change of carotid artery quantitatively and early in postmenopausal women with OP.
