256层螺旋CT血管成像在主动脉夹层诊断中的价值

摘　　要：目的探究256层螺旋CT血管成像（CTA）对主动脉夹层（AD）的临床诊断价值。方法选择70例疑似AD患者为研究对象，所有患者均行256层CTA检查，对图像信息进行数据重建后处理分析，分析AD患者的CT影像学特征及诊断价值。结果256层CTA共检出AD 66例（97.06%），其中DeBakey Ⅰ型20例，Ⅱ型3例，Ⅲ型43例；Stanford A型23例，B型43例。66例AD患者CTA检查均能清晰显示内膜片及真假腔结构，真腔CT值明显高于假腔（P〈0.05）；CTA显示66例AD患者均显示有初始破口，其中62例有1个或多个再破口；此外，CTA能清晰显示夹层累及血管范围、内膜片情况、假腔内血栓形成及并发症等。结论256层CTA诊断AD快速、安全、准确性高，是AD筛查及诊断的可靠方法，值得推荐使用。ObjectiveTo investigate the clinical diagnostic value of 256 slice spiral CT angiography （CTA） for aortic dissection（AD）.MethodsSeventy patients suspected with AD were selected as research objects, and all patients were examined with 256 layers of CTA. Then, the image information were processed and analyzed after data reconstruction, and the CT image features and diagnostic values of AD were observed.Results256 layers of CTA checked out 66 cases （97.06%） of AD, including 20 cases of DeBakey I, 3 cases of DeBakey Ⅱ and 43 cases of DeBakey Ⅲ, 23 cases and 43 cases of Stanford A, B type. The CTA tests for all 66 AD patients were able to clearly display the structure of endometrium and the true and false cavity, and the true cavity CT value was significantly higher than that in the false cavity （P〈0.05）. CTA showed that 66 patients all had initial breach, of which 62 cases had one or more breaks. In addition, CTA can clearly show the range of vascular interlayer, situation of endometrium, formation of thrombus in the false cavity and the complications.ConclusionsThe 256 layer CTA in diagnosis of AD is fast, safe and accurate, and it is a reliable method for AD screening and diagnosis, thus it is worthy of recommending and application.
