摘  要:目的探讨超声弹性成像联合BI-RADS分级系统对乳腺肿物的临床诊断价值。方法对术前常规彩色多普勒超声检查未排除恶性可能的157例患者(189个乳腺肿物)的临床资料进行回顾分析,采用单用BI-RADS分级、单用超声弹性成像及超声弹性成像联合BI-RADS分级分别评估乳腺肿物的性质。所有乳腺肿物均行病理活检明确诊断作为金标准,分析并比较三种检查方法的临床诊断价值。结果术后病理检查结果显示,189个乳腺肿物中,恶性58个(30.69%),良性131个(69.31%);BI-RADS分级系统诊断的准确率为85.19%,超声弹性成像的诊断准确率为89.42%,联合检查法的准确率为94.18%,联合检查法在乳腺肿物的诊断灵敏度、特异度及准确性上均优于单用BI-RADS法或单用超声弹性成像法(P〈0.05)。结论采用超声弹性成像联合BI-RADS分级有利于提高乳腺肿物诊断的准确性,值得在临床上推广。ObjectiveTo evaluate the diagnostic value of ultrasound elastography combined with BI-RADS grading system for breast tumors.MethodsClinical data of 157 patients (189 breast tumors) who were suspected with malignant tumor by preoperative color Doppler ultrasoundgraphy were retrospectively analyzed. The properties of the breast tumor were evaluated by BI-RADS grading, ultrasound elastography and ultrasound elastography combined with BI-RADS classification. All breast tumors were detected by pathological biopsy, of which results were considered as the golden standard. The clinical diagnostic value of the above three methods was statistically analyzed and compared.ResultsThe results of postoperative pathological examination showed that among the 189 breast tumors, 58 (30.69%) cases were malignant, 131 (69.31%) cases were benign. The accuracy rate of BI-RADS classification system was 85.19%, the diagnostic accuracy of ultrasound elastography was 89.42%, the accuracy rate of combined examination was 94.18%. The sensitivity, specificity and accuracy of combined method were better than those of single BI-RADS or ultrasound elastography (P〈0.05).ConclusionsUltrasound elastography combined with BI-RADS grading can improve the diagnostic accuracy for breast tumor, and it is worthy of clinical promotion.
