摘 要:目的探索盐城市肺结核就诊延迟的季节性特征,为盐城市肺结核发现工作提供依据。方法从结核病管理信息系统中收集2009—2015年盐城市就诊延迟的肺结核患者数据,根据季节性分析结果,分上、下半年采用改良圆形分布法进行分析。结果盐城市肺结核就诊延迟1—12月份的季节指数分别为:81.12%、79.84%、123.51%、109.55%、107.39%、98.83%、92.78%、101.01%、101.74%、95.38%、114.87%、93.99%。3—5月、8—9月及11月为就诊延迟旺季。经圆形分布雷氏z值检验,上下半年高峰日分别为4月9日和10月4日,高峰期分别为4月3—16日和9月6日—10月31日。2009—2015年,上下半年分别有6年、5年的平均角峰值有统计学意义(P<0.01),上半年高峰日和高峰期均在3—4月,下半年相对变动较大。结论采用改良的圆形分布法进行分析盐城市肺结核就诊延迟的季节性规律,具有较好的实用性,可有效指导肺结核发现工作。[Objective]To explore the seasonal characteristics of health-seeking-delay of pulmonary tuberculosis(PTB) patients in Yancheng City,provide the basis for the discovery of PTB. [Methods] The data of PTB patients with health-seeking-delay in Yancheng City from 2009-2015 were collected from the tuberculosis management information system. According to the results of seasonal analysis,the improved circular distribution method was applied in the investigation of the first half and the second half of the year. [Results] From January to December,the seasonal index of PTB health-seeking-delay in Yancheng City was 81.12%,79.84%,123.51%,109.55%,107.39%,98.83%,92.78%,101.01%,101.74%,95.38%,114.87% and 93.99%,respectively. March to May,August to September and November were the peak of PTB health-seeking-delay. The circular distribution z test showed that peak day in the first half and the second half of the year was 9 April and 4 October respectively,while the peak period was3-16 April and 6 September to 31 October respectively. During 2009-2015,differences in peak values of mean angle were statistically significant in 6 first half and 5 second half of the year(P〈0.01). The peak days and peak periods in the first half of the year were all in March and April,while these were relatively diverse in the second half of the year. [Conclusion] The improved circular distribution method has good practicability in the analysis of the seasonal rule of PTB health-seeking-delay in Yancheng City,and could effectively guide PTB discovery.
