米力农治疗老年心力衰竭合并肺动脉高压患者的临床疗效观察

摘　　要：目的观察米力农治疗老年心力衰竭(心衰)合并肺动脉高压患者的临床疗效。方法选取医院2016年1月-2017年4月收治的老年心衰合并肺动脉高压患者80例,随机分为观察组和对照组各40例,对照组应用常规抗心力衰竭治疗,观察组在对照组的基础上应用米力农注射液进行治疗,观察2组临床疗效,心率(HR)、收缩压(SBP)、舒张压(DBP)、左心室射血分数(LVEF)、氨基末端脑钠肽前体(NT-proBNP)以及肺动脉收缩压(PAP)的变化情况,并观察2组患者的不良反应发生情况。结果观察组总有效率为95.00%显著高于对照组的85.00%;治疗后2组患者的心率、舒张压、肺动脉高压低于治疗前,收缩压及LVEF高于治疗前,且观察组变化幅度大于对照组;治疗后7 d、14 d 2组患者的NT-proBNP水平均低于治疗前,且观察组患者的NT-proBNP水平均低于对照组,差异均有统计学意义(P<0.05)。结论应用米力农治疗老年心衰合并肺动脉高压患者有一定的临床效果,值得推广。Objective To observe the clinical effect of milrinone on elderly patients with heart failure and pulmonary hypertension. Methods 80 elderly patients with heart failure and pulmonary hypertension,selected in the hospital from January 2016 to April 2017,were randomly divided into observation group and control group,each of 40 cases. Control group was treated with routine anti-heart failure,observation group was treated with milrinone injection on the basis of control group. Observed two groups of clinical efficacy,heart rate（ HR）,systolic blood pressure（ SBP）,diastolic blood pressure（ DBP）,left ventricular ejection fraction（ LVEF）,aminoterminal probrain natriuretic peptide（ NT-proBNP） and pulmonary systolic pressure（ PAP） changes in the situation and observed the incidence of adverse reactions in two groups. Results The total effective rate in observation group was 95. 00%,which was significantly higher than 85. 00% in control group; After treatment,the heart rate,diastolic blood pressure and pulmonary hypertension in two groups were lower than before treatment,systolic blood pressure and LVEF were higher than before treatment,and observation group had a greater change than control group; At 7 d and 14 d after treatment,the NT-proBNP levels in two groups were lower than before treatment,and the NT-proBNP levels in observation group were lower than those in control group,the differences were statistically significant（ P 〈0. 05）. Conclusion The application of milrinone in the treatment of elderly patients with heart failure and pulmonary hypertension has a certain clinical effect and is worth popularizing.
