股—腘—小腿动脉长段闭塞行导管接触性溶栓辅助腔内血管成形术治疗的效果观察

摘　　要：目的观察导管接触性溶栓在股—腘—小腿动脉长段闭塞腔内治疗中的效果。方法选择医院2011年6月-2015年6月收治的股—腘—小腿动脉长段闭塞且行导管接触性溶栓辅助腔内血管成形术治疗的患者23例,术后给予抗凝、溶栓治疗,术前、术后给予人文关怀及健康指导,出院后给予电话随访。结果所有患者术后临床症状均明显改善,踝肱指数由术前的0.19±0.10显著增加到术后的0.77±0.24(P<0.05)。导管接触性溶栓治疗期间发生穿刺处鞘管周围出血3例,立即处理后好转。结论导管接触性溶栓辅助腔内血管成形术治疗股—腘—小腿动脉长段闭塞的效果好,且安全性好,正确的护理能减少并发症的发生,促进患者早日康复。Objective To observe the effect of catheter-directed thrombolysis in the treatment of long occlusive femoral-popliteal-lower leg artery. Methods 23 patients with long occlusive femoral-popliteal-lower leg artery from June 2011 to June 2015 in the hospital were selected,and they were treated with catheter-directed thrombolysis assisted endovascular angioplasty,postoperative anticoagulation and thrombolytic therapy were given,humane care and health guidance were given before and after surgery,telephone follow-up was given after discharge. Results All cases of postoperative clinical symptoms were improved significantly,ABI was obviously increased from preoperative 0. 19 ± 0. 10 to postoperative 0. 77 ± 0. 24（ P〈 0. 05）.During thrombolytic therapy with surrounding piercing sheath pipe place bleeding occurred in 3 patients,immediately after processing better. Conclusion Catheter-directed thrombolysis assisted endovascular angioplasty is effective and safe treatment for long occlusive femoral-popliteal-lower leg artery,and correct nursing can reduce complications,promote the patient recover soon.
