摘 要:目的探讨MSCT薄层最大密度投影(max imumintensity projection,MIP)在鼻骨骨折法医鉴定中的应用价值,提高鼻骨骨折鉴定的可信度。方法对60例鼻外伤进行法医临床鉴定的患者和60例无鼻外伤史人群进行MSCT扫描,并分析CT平扫和MSCT薄层MIP评价是否存在鼻骨骨折以及骨折类型的检查结果。结果Ⅰ型鼻骨骨折、Ⅱ型鼻骨骨折、Ⅲ型鼻骨骨折、Ⅳ型鼻骨骨折在CT平扫中的阳性率分别为88.8%、100%、100%、88.8%;在薄层MIP中的阳性率分别为100%、100%、100%、100%。结论 MSCT薄层MIP较CT平扫能获得更加立体直观的鼻骨薄层图像,准确判断鼻骨骨折的位置、范围、类型和程度,为鼻骨骨折的法医学鉴定提供可靠依据。Objective To explore the application value of maximum intensity projection of multi-slice spiral computed tomography( MSCT) in nasal bone fracture and improve the accuracy of forensic identification. Methods Sixty patients with nasal bone fracture and 60 healthy individuals( Control? group) were examined by means of MSCT. The results of conventional CT scan and MST thin-layer MIP were analyzed and to confirm whether there is nasal bone fracture and the type of nasal bone fracture. Results The positive rates of type I nasal bone fracture,type II nasal bone fracture,type III nasal bone fracture and type IV nasal bone fracture were 88. 8%,100%,100% and 88. 8% respectively in conventional CT scan,and were 100%,100%,100%,100% respectively in MSCT thin-layer MIP. Conclusions MSCT thin-layer MIP can get a more stereoscopic and intuitive nasal bone thin-layer image than conventional CT scan,accurately determine the location,range,type and degree of nasal bone fracture,and provide a reliable basis for forensic identification of nasal bone fracture.
