陈竞纬教授三要四辨法膏方调治高血压病

摘　　要：[目的]探讨陈竞纬教授三要四辨法膏方调治高血压病的经验。[方法]通过查阅文献、跟师临诊,总结陈竞纬教授三要四辨法膏方调治高血压病之学术经验。包括膏方调治高血压病之适应证,三要四辨法的具体内涵,并附验案一例。[结果]陈师认为,膏方调治高血压病有其独特适应证。对于适合使用膏方调治的高血压病患者,治疗时一要填补下元以涵养肝木,祛除实邪而无致眩之由;二要健运中州以防风木乘土,调畅气机再复升降之圆;三要整体调治以纠脏腑之偏,以顺为补,最忌漫投补药。辨证时首辨本虚之偏颇,次辨患者标实之偏胜,再辨患者饮食、睡眠、二便等身体一般情况,最后辨患者的兼夹疾病。治疗上使用药对为君,辅以臣佐,扩展成方。所附验案一则,予以佐证。[结论]临床实践证明,陈竞纬教授三要四辨法膏方调治高血压病疗效显著,可操作性强,值得借鉴和推广应用。[Objective] To explore Professor CHEN Jingwei＇s experience in treating hypertension with herbal paste by the method of ＂San Yao Si Bian＂.[Methods] By consulting relevant literatures and following the teacher clinic, the author summarizes Professor CHEN Jingwei＇s opinions about the method of＂San Yao Si Bian＂in treating hypertension with herbal paste,which includes the indications of herbal paste in the treatment of hypertension,the meaning of＂San Yao Si Bian＂method. A proven case is introduced by the author.[Results]Herbal paste has unique indications in the treatment of hypertension patients.When treating for suitable patients with hypertension, Professor CHEN believes that three main points which are ＂tonifying the kidney to calm the liver and suppress Yang, eliminating pathogenic factors to treat vertigo＂,＂strengthening the middle-Jiao to avoid the aggression of the liver, regulating the body＇s Qi to prevent it unobstructed＂, ＂treating integrally to rectify the deviation of viscera, avoiding misusage of tonic＂must be considered. When prescribing for appropriate patients with hypertension, Professor CHEN thinks that doctors should discriminate the bias of deficiency firstly, distinguish the excession of the evil secondly, then enquire the patients＇ diet, sleep, stool, urine and other general physical conditions, finally ask patients if they have any other diseases excluding hypertension. In Professor CHEN＇s opinions, the couplet medicines should be chosen to be the monarch drugs in the herbal paste, then add the minister, adjuvant, and guide herbs to expand the prescriptions of the herbal paste. One case is attached to support the CHEN＇s opinions. [Conclusion]Professor CHEN Jingwei＇s method of＂San Yao Si Bian＂has good clinical efficacy and maneuverability,which should be popularized and used for reference.
