摘　　要：目的采用密切值法综合评价天津市医疗机构法定传染病报告质量,为从不同层面提高全市报告质量提供科学依据。方法查阅2016年1—11月医疗机构门诊日志、出入院登记簿,抄录初步诊断为法定传染病的病例,依据医疗机构等级、地域和类别分层,应用密切值法对计算出的法定传染病报告率、报告及时率、报告卡填写完整率、填写准确率、网络报告信息一致率进行报告质量综合评价。结果调查1 024例病例,总报告率98.63%,报告及时率98.51%,报告卡填写完整率99.90%,填写准确率94.25%,网报一致率96.14%,密切值法对不同等级、地域、类别医疗机构报告质量综合评价后排序结果分别是一级>三级>二级,市区>近郊>远郊,社区卫生服务中心>综合型>中医型>传染病型。结论密切值法综合评价结果反映出天津市医疗机构法定传染病报告质量呈现不同层次水平,建议加大二级、远郊、传染病型及中医型医疗机构督导力度,以提高报告质量。[ Objective ] To synthetical evaluate the notifiable communicable disease reporting quality of medical institutions in Tianjin by Close-value method, and provide scientific evidence for improvement of reporting quality. [Methods] The survey was conducted among medical institutions from January to November, 2016 by inspecting outpatient records and admission register, and recording preliminarily diagnosed cases. The reporting rate, timeliness rate, completeness rate, accuracy and concordance rate were calculated to synthetically evaluate reporting quality among different classes, regions and types of medical institutions by close-value method. [Results] A total of 1 024 cases were investigated, with the overall reporting rate, timeliness rate, completeness rate, accuracy and concordance rate of 98.63%, 98.51%, 99.90%, 94.25%, 96.14%, respectively. Medical institutions reporting quality by Close-value method for different classes, regions and types from high to low were first class, third class, second class; downtown area, suburb, outer suburb; community health centre, comprehensive, traditional Chinese medicine, infectious disease institution. [Conclusion] Close-value method reflects the notifiable communicable disease reporting quality in Tianjin presents various level. It is suggested to strengthen the supervision especially in second class, outer suburb, infectious disease and traditional Chinese medicine medical institutions, so as to improve the overall reporting quality.
