摘  要:目的观察新型强脉冲光治疗面部毛细血管扩张症的疗效,及其对血管内皮生长因子(VEGF),转化生长因子(TGF)-β1水平的影响。方法选择2015年1月~2016年12月在我院皮肤科门诊就诊的面部毛细血管扩张症患者60例。按照随机数字法将患者分为观察组和对照组,每组各30例。观察组采用新型强脉冲光治疗,对照组采用脉冲染料激光治疗。观察两组的疗效和不良反应率,治疗前和治疗后观察组皮肤黑素指数(MI)、红斑指数(EI)、经皮水分丢失(TEWL)、VEGF和TGF-β1的变化。结果治疗后观察组的总有效率为90.00%,对照组的总有效率为93.33%,两组的疗效差异无统计学意义P>0.05)。观察组的不良反应发生率为13.33%明显低于对照组的46.67%(P<0.05)。治疗前两组的MI、EI、TEWL、VEGF和TGF-β1水平差异无统计学意义(P>0.05),治疗后两组的水平均较治疗前明显降低(P<0.01),观察组的皮肤MI、TEWL、VEGF和TGF-β1较对照组降低更为明显(P<0.01),而EI两组水平差异无统计学意义(P>0.05)。结论新型强脉冲光治疗面部毛细血管扩张症的疗效确切,具有不良反应少,对色素沉着和水分保留方面具有优势,可能与血管相关发育蛋白的水平有关。Objective To investigate the efficacy of new intense pulsed light in the treatment of facial telangiectasia, and impact on the levels of vascular endothelial growth factor (VEGF) and transforming growth factor(TGF) beta I. Methods 60 patients with facial telangiectasia, from January 2015 to December 2016, were divided into observation group and control group according to the random number method, each group had 30 cases. The observation group was treated with new intense pulsed light, the control group was treated with pulsed dye laser. The curative effect and adverse reaction rate were ob- served in two groups after treatment. The levels of skin melanin index (MI) , erythema index (El) , transepidermal water loss ( TEWL ) ,VEGF and TGF- beta 1 were detected before and after treatment Results After treatment, the total effective rate was 90% in observation group, 93.33% in control group, there was no significant effect between the two groups (P 〉 0.05). The adverse reaction rate was 13.33% in observation group,which was significantly lower than 46.67% in control group (P 〈 0.05). Before treatment, the levels of the skin melanin index, erythema index, transepidermal water loss, VEGF and TGF- beta 1 were no statistical significance between two groups (P 〉 0.05), after treatment, the levels of those were significantly lower in two groups than those before treatment (P〈 0.01), and the decreasing levels of skin melanin index, transepidermal water loss, VEGF and TGF- beta t in observation group were more obvious compared with the control group (P 〈 0.01), but there were no significant differences of erythema index between two groups (P 〉 0.05) Conclusion The efficacy of new intense pulsed light in the treatment of facial telangiectasia was obvious, it has the advantages of less adverse reaction, pigmentation and water retention, its mechanism may be related to vascular development related protein.
