双叶皮瓣在面部皮肤缺损修复中的应用观察

摘　　要：目的观察面部皮肤缺损应用双叶皮瓣修复对提高手术有效率及降低不良事件率的影响。方法选取2013年2月至2017年12月收治的26例面部皮肤缺损患者临床资料，均接受双叶皮瓣修复治疗，观察修复效果及术后恢复、满意度情况。结果经术后1个月随访，26例患者修复优良率为92．31％（24／26），其中优18例（69．23％）、良6例（23．08％）、差2例（7．69％）。26例患者切口均Ⅰ期愈合，皮瓣均成活，血运良好，术后感染1例、血肿1例，无坏死、明显瘢痕等并发症，且肿瘤患者无转移及复发，并发症约为7．69％（2／26）。修复总满意度为96．15％（25／26），其中满意20例、一般5例、不满意1例。结论面部皮肤缺损应用双叶皮瓣修复的效果显著，能提高修复优良性，且术后皮瓣恢复良好，并发症少，患者满意度高，具推广价值。Objective To observe the effect of bilobed flap repair on facial skin defects in improving the efficiency of surgery and reducing the incidence of adverse events. Methods The clinical data of 26 patients with facial skin defects treated from February 2013 to December 2017 were selected, who were treated with bilobed flap repair. Then observed the repair effect, postoperative recovery and satisfaction. Results After one month of follow-up, the excellent and good rate of repair in 26 patients was 92.31% （24/26）, of whom 18 cases were excellent （69.23%）, 6 cases were good （23.08%）, and 2 cases were poor （7.69%）. All incisions were healed by first intention in all 26 patients. The flaps survived with good blood supply. There were 1 case of postoperative infection and 1 case of hematoma, without complications such as necrosis, obvious scars, or tumor metastasis and recurrence. The complication rate was 7.69% （2/26）. The total satisfaction of repair was 96.15% （25/26）, of whom 20 cases were satisfactory, 5 cases were general, and 1 case was not satisfactory. Conclusions The application of bilobed flap is effective in the repair of facial skin defects, and can improve the repair quality. The flaps are well restored after the operation, with few complications and high patient satisfaction, worthy of promotion.
