摘 要:目的探讨电针对脓毒症大鼠炎症反应和器官损伤的影响,分析其与胆碱能抗炎通路中枢迷走神经背核的关系。方法成年雄性SD大鼠96只,按照随机数字表法分为对照组、电针组、电针+半胆碱基-(HC-3)组,每组各32只。3组大鼠均于术前1周采用脑立体定位仪引导,参照大鼠立体脑定位图谱行迷走背核定位,建立注射针固定套管准备给药。所有大鼠均采用盲肠结扎穿孔术(CLP)制备脓毒症大鼠模型;对照组仅给予造模处理,其余两组造模成功前对双侧足三里行电针治疗,电针+HC-3组在电针治疗双侧足三里前30 min,固定套管注射1μl HC-3。3组均取20只大鼠用于观察术后7 d存活率。各组于术后24 h,取其中6只的部分肺、肝组织观察病理学改变及损伤评分;剩余6只大鼠眼球取血,检测肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)和白细胞介素-6(IL-6),取肺检测高迁移率族蛋白1(HMGB1)。结果 CLP术后第7天,电针组和电针+HC-3组大鼠的存活率明显高于对照组,差异均有统计学意义(P<0.05);电针和电针+HC-3组比较,差异无统计学意义(P>0.05)。与对照组比较,电针组和电针+HC-3组大鼠肺、肝组织病理损伤减轻,组织病理学损伤评分及血清中炎症因子水平均降低,差异均有统计学意义(P<0.05);而电针组和电针+HC-3组大鼠肺、肝组织病理损伤情况、组织病理学损伤评分及血清中炎症因子水平比较,差异均无统计学意义(P>0.05)。结论电针可以减轻脓毒症大鼠炎症反应及器官损伤,且这一过程不依赖于迷走神经背核。Objective To investigate the effect of electro-acupuncture on inflammatory reaction and organ injury of sepsis rats and its relation with dorsal motor nucleus of vagus. Methods A total of 96 male SD rat were randomly divided into groups( n = 32) : control group,electro-acupuncture( EA) group and EA + HC-3 group. To set up the injection needle to prepare the medicine,dorsal motor nucleus of the vagus( DMN) of all rats were located by use of stereotaxic apparatus according to three-dimensional brain mapping of rats one week before molding. The sepsis was induced by cecal ligtion and puncture( CLP); the rats in control group were performed the same operation with CLP. The rats in EA group and EA + HC-3 group were treated with EA before molding. The rats in EA + HC-3 group were injected with HC-3 30 minutes to block cholinergic anti-inflammation pathway before EA( HC-3,inhibitor of choline acetylase,which can control production of choline acetylase and deactivate acetylcholine receptors in dorsal motor nucleus of the vagus).The survival rats were observed during 7 days in 20 rats of each group. Part lung and liver tissue were harvested to test histopathology and pathologic score in 6 rats of each group at 24 hours after CLP in six rats of each group. Then eyeball blood were managed for detection of the concentration of tumor necrosis factor-( TNF-α),interleukin-1β( IL-1β),interleukin-6( IL-6) and part lung for high mobility group box 1( HMGB1) at 24 hours after CLP in the remaining 6 rats of each group. Results At day 7 of CLP,compared with the control group,the survival rate in EA group and EA + HC-3 group significantly increased( P〈0. 05),there was no statistically significant difference between EA group and EA + HC-3 group( P 0. 05). Compared with the control group,the injury of lung and liver tissue reduced,histopathological lesion scores and serum levels of inflammatory factors reduced in EA group and EA + HC-3 group( P〉0. 05).
