摘 要:目的研究胆红素的变化与新生儿黄疸严重程度和肝功能的相关性。方法选取2014年12月至2016年12月北京太和妇产医院收治的黄疸新生儿200例,根据不同严重程度和病因进行分组。检测并比较不同临床特征患儿血清总胆红素及非结合胆红素水平和肝功能水平,并分析黄疸患儿血清总胆红素及非结合胆红素水平与肝功能的相关性。结果重度黄疸、有核黄疸、溶血性黄疸患儿的总胆红素和非结合胆红素水平分别高于中度黄疸、无核黄疸和感染性黄疸患儿(t值分别为-7.586、3.251、-1.718、-4.861、2.171、-1.706,均P<0.05)。重度黄疸、有核黄疸、溶血性黄疸患儿的谷丙转氨酶(ALT)、谷草转氨酶(AST)水平高于中度黄疸、无核黄疸和感染性黄疸患儿(t值分别为3.251、2.171、11.959、5.506,-1.718、-1.706、-11.822、-13.596,均P<0.05)。黄疸患儿总胆红素和非结合胆红素水平与ALT、AST水平明显正相关(r值分别为0.468、0.512、0.532、0.498,均P<0.05)。结论血清总胆红素及非结合胆红素水平有助于对新生儿黄疸病情严重程度的诊断和评估。Objective To study the correlation between bilirubin and the severity of neonatal jaundice as well as liver function.Methods Two hundred cases of neonatal jaundice in Beijing Taihe Maternity Hospital from December 2014 to December 2016 were selected and grouped according to different severity and etiology.The serum levels of total bilirubin,unconjugated bilirubin and liver function were measured and compared.The correlation between serum total bilirubin and unconjugated bilirubin levels and liver function was analyzed.Results The levels of total bilirubin and unconjugated bilirubin in cases with severe jaundice,nuclear jaundice and hemolytic jaundice were higher than those in cases with moderate jaundice,no nuclear jaundice and infectious jaundice(t value was-7.586,3.251,-1.718,-4.861,2.171 and-1.706,respectively,all P〈0.05).The levels of ALT and AST in cases with severe jaundice,nuclear jaundice and hemolytic jaundice were higher than those with moderate jaundice,no nuclear jaundice and infectious jaundice(t value was 3.251,2.171,11.959,5.506,-1.718,-1.706,-11.822 and-13.596,respectively,all P〈0.05).The levels of total bilirubin and unconjugated bilirubin in cases with jaundice were positively correlated with ALT and AST(r value was 0.468,0.512,0.532 and 0.498,respectively,all P〈0.05).Conclusion Detection of serum total bilirubin and unconjugated bilirubin levels is helpful for the diagnosis and evaluation of the severity of neonatal jaundice.
