鼻内针刺联合益气解敏汤治疗中重度变应性鼻炎肺脾气虚型60例临床观察

摘　　要：目的评价鼻内针刺联合自拟益气解敏汤治疗中重度变应性鼻炎肺脾气虚型的临床疗效。方法纳入北京中医药大学东直门医院中重度变应性鼻炎肺脾气虚型患者60例,采用鼻内镜引导下针刺双侧鼻腔内迎香穴和鼻丘穴,隔日1次;同时口服益气解敏汤,每日1剂,共治疗2周。分别在治疗前后对患者进行视觉模拟量表(VAS)、鼻炎主要症状评分(TNSS)、一般活动的鼻结膜炎生命质量调查问卷(RQLQ)、鼻腔体征评分、鼻腔分泌物嗜酸细胞(EOS)计数临床指标评价。结果治疗后60例患者VAS、TNSS、RQLQ、鼻腔体征评分均较治疗前明显降低,鼻腔分泌物EOS计数均较治疗前减少,差异均有统计学意义(P<0.01)。结论鼻内针刺联合益气解敏汤治疗中重度变应性鼻炎肺脾气虚型疗效显著,可作为变应性鼻炎的补充或替代疗法之一。Objective To evaluate the clinical efficacy of intranasal acupuncture combined with self-made Yiqi Jiemin Decoction（ 益 气解敏汤） in the treatment of moderate-severe allergic rhinitis. Methods A total of 60 patients diagnosed as moderate to severe allergic rhinitis with spleen-qi deficiency were collected at the Dongzhimen Hospital of Beijing University of Traditional Chinese Medicine. Both sides of Neiyingxiang（ EX-HN9） and Biqiu（ EX-HN） acupoints were acupunctured by endoscopic guidance,once every other day; simultaneous oral administration of Yiqi Jiemin Decoction,one dose per day for a total of 2 weeks. Visual analogue scales（ VAS）,rhinitis primary symptom scores（ TNSS）,general rhinitis conjunctivitis quality of life questionnaire（ RQLQ）,nasal signs,and nasal secretion eosinophils（ EOS） were performed before and after treatment. The clinical indicators were measured. Results After treatment,the scores of VAS,TNSS,RQLQ,and nasal signs were significantly lower than before treatment in 60 patients. EOS counts of nasal secretions decreased compared with those before treatment,and the difference was statistically significant（ P〈0. 01）. Conclusion Intranasal acupuncture combined with Yiqi Jiemin Decoction is effective in the treatment of moderate-severe allergic rhinitis with deficiency of lung and spleen qi. It can be used as a supplement or alternative therapy for allergic rhinitis.
