摘 要:目的探讨高迁移率族蛋白(HMG)B1、Toll样受体(TLR)4在脓毒症大鼠心肌组织中表达及意义。方法选取清洁级SD大鼠50只,随机分为正常对照组、假手术组和模型组,其中模型组再分为模型12、24、48 h组,每组10只,采用盲肠结扎穿孔术制备脓毒症大鼠模型,采用免疫组化染色检测心肌组织HMGB1表达,酶联免疫吸附检测TLR4表达,实时荧光定量聚合酶链反应(PCR)检测HMGB1和TLR4 mRNA表达。结果正常对照组和假手术组心肌基本正常,模型12、24、48 h组可见心肌细胞水肿,炎症细胞浸润,间质血管充血,心肌细胞坏死明显;模型12、24、48 h组心肌组织HBGB1的OD值、TLR4及HBGB1 mRNA和TLR4 mRNA相对表达量明显高于正常对照组和假手术组(P<0.05),其中模型24 h组心肌组织HBGB1的OD值、TLR4及HBGB1mRNA和TLR4 mRNA相对表达量分别为0.846±0.103、(24.18±4.10)ng/ml、3.511±0.414和3.671±0.501,明显高于模型12、48 h组(P<0.05);HMGB1 mRNA和TLR4 mRNA相对表达量呈正相关(r=0.611,P<0.05)。结论 HMGB1和TLR4在脓毒症大鼠心肌组织中表达上调,可能在脓毒症心肌损伤中有重要作用。Objective To investigate the expression and significance of high mobility group protein (HMG) B1 and Toll like recep- tor (TLR) 4 in the myocardial tissue of sepsis rats. Methods 50 clean SD rats were selected and randomly divided into normal control, sham operation and model group, the model group was further divided into model 12,24 and 48 h group, with 10 rats in each group, model of sepsis rat was prepared by cecal ligation and perforation, HMGB1 expression in myocardial tissue was detected by immunohistochemical staining, TLR4 expression was detected by enzyme-linked immunosorbent assay, and HMGB1 and TLR4 mRNA were detected by real-time fluorescence quantitative PCR. Results In normal control and sham operation group,the myoeardium of rats were basically normal,in model 12,24 and 48 h group, myocardial cell edema, infihration of inflammatory cells, congestion of interstitial vessels, and necrosis of myocardial cells were ob- served. In model 12,24,48 h groups, myocardial HBGB10D values, TLR4, HBGB1 mRNA and TLR4 mRNA relative expression were signifi- cantly higher than those of normal control and sham operation group (P〈0.05), HBGB10D values ,TLR4, HBGBI mRNA and TLR4 mRNA expression levels in myocardial of model 24 h group were 0. 846±0. 103, (24.18 ±4.10) ng/ml,3.511 ±0. 414 and 3.671 ±0.501, significant- ly higher than those of model 12,48 h group (P〈0, 05 ). There was a positive correlation between the relative expression of HMGB1 mRNA and TLR4 mRNA (r=0. 611 ,P〈0.05 ). Conclusions The up-regulated expression of HMGB1 and TLR4 in the myocardium Of sepsis rats may play an important role in myocardial injury of sepsis.
