摘  要:目的探究12岁儿童能量代谢率及其与身体成分的关系,为儿童生长发育研究提供基础。方法在上海市某中学招募30名12岁儿童(男童18名,女童12名),采用双能X线仪(DEXA)测量身体成分,包括体脂率(%)、脂肪量(kg)、瘦体重(kg)和肌肉量(kg)。采用能量代谢仓测量受试者静息代谢率(RMR)、睡眠代谢率(SMR)和基础代谢率(BMR)。结果校正体重后,儿童的餐后2 h RMR(RMR-2 h)[(0.026±0.004)kcal/(kg·min^(-1))]高于餐后4 h RMR(RMR-4 h)[(0.023±0.004)kcal/(kg·min^(-1))]和BMR[(0.023±0.004)kcal/(kg·min^(-1))],SMR[(0.019±0.003)kcal/(kg·min^(-1))]低于BMR(P值均<0.05)。RMR-2 h,RMR-4 h和SMR分别与BMR呈正相关(r值分别为0.603,0.657,0.870,P值均<0.05)。男生中,RMR-2 h与BMR的相关性高于RMR-4 h与BMR的相关性,男生RMR-4 h与BMR,SMR与BMR的相关性均低于女生。男生的BMR分别与体重、BMI、脂肪量、瘦体重、肌肉量呈正相关(r值分别为0.737,0.553,0.470,0.597,0.598,P值均<0.05),女生的BMR仅与瘦体重、肌肉量相关(r值分别为0.718,0.723,P值均<0.05)。结论采用RMR-4 h预测儿童的BMR较RMR-2 h更准确。总体水平上可采用BMR预测SMR,但需注意个体间差异。儿童RMR,SMR,BMR之间及能量代谢率与身体成分具有相关性,性别间有差异。Objective The aim of this study was to investigate the energy metabolic rate and the correlation between energy metabolic rate and body composition among 12-year-old children, and to provide a practical basis for measurement of children's energy metabolism. Methods Thirty children aged 12 years old( 18 boys and 12 girls) were recruited. Body composition, including fat rate( %), fat mass( kg), lean body mass( kg) and muscle mass( kg), was measured by Lunar Dual Energy X-Ray Absorptiometry( DEXA). Resting metabolic rate( RMR-2 h and RMR-4 h), sleeping metabolic rate( SMR) and basal metabolic rate( BMR)were measured by the metabolic chamber. Results After rectification of body weight, RMR-2 h[0.026±0.004 kcal/( kg·min^-1) ]was significantly higher than RMR-4 h[0.023±0.004 kcal/( kg·min^-1) ] and BMR[0.023±0.004 kcal/( kg·min^-1) ], respectively( P〈0.05), and SMR[0.019±0.003 kcal/( kg·min^-1) ] was significantly lower than BMR( P〈0. 05). RMR-2 h, RMR-4 h,SMR were positively correlated with BMR( r = 0. 603,0. 657,0. 870,P〈0. 05). The correlation between RMR-2 h and BMR was higher than that between RMR-4 h and BMR in boys. The correlation among RMR-4 h and BMR, SMR and BMR in boys was lower than that in girls. In boys, BMR was positively correlated with weight, BMI, fat mass, lean fat mass and muscle mass( r =0.737,0.553,0. 470,0. 597,0. 598, P〈0. 05). In girls, BMR was positively correlated with lean fat mass and muscle mass( r =0.718,0.723,P〈0.05). Conclusion For children, RMR-4 h was predictive of BMR. In general, BMR was predictive of SMR,but the individual difference should be considered. There was different correlation among RMR, SMR and BMR in different genders.There is correlation between energy metabolic rate and body composition, and the correlation was different in different genders.
