中国大陆学生男男性行为及HIV感染检出率的Meta分析

摘　　要：目的了解中国大陆学生男男性行为人群(men who have sex with men,MSM)中性行为、HIV检测与艾滋病病毒感染及其时间和地区分布状况,为制定有针对性的干预措施提供依据。方法在中国知网、维普、万方和Pub Med数据库中,通过相应检索策略查找2012年1月1日至2017年6月30日期间公开发表的针对中国大陆学生MSM开展的有关HIV感染率、危险性行为、HIV检测率的研究文献,整理后提取相关数据进行Meta分析。结果共有20篇文献入选,累计MSM学生6 286名。结果表明,在2012—2016年间中国大陆有男男性行为的学生中,HIV感染的合并率高达5.2%(95%CI=4.0%~6.5%),最近6个月每次使用安全套和最近1年曾做过HIV检测的报告率分别为35.5%(95%CI=23.8%~47.2%)和44.3%(95%CI=34.0%~54.6%)。研究未发现明显发表偏倚。结论学生MSM人群艾滋病高流行、安全套坚持使用率低及HIV检测率低的状况应引起艾滋病预防干预研究的高度关注。Objective To know the status of sexual behavior, HIV testing and HIV infection among male students who have sex with men（ MSM students） in Chinese mainland, and to provide reference for targeted interventional measures. Methods Literature searches were systematically performed by using electronic databases（ Pub Med, Chinese National Knowledge Infrastructure, Wanfang and VIP database） with date restriction from January 1, 2012 to June 30, 2017. Then the relevant data was extracted, and transformed into a database for Meta-analysis. Results A total of 20 studies were included in this Meta-analysis. There were 6 286 MSM students enrolled in these studies, and the Meta-analyses showed that the overall or pooled prevalence of HIV infection was 5.2%（ 95%CI = 4.0%-6.5%）, the rates of condom usage at every sexual intercourse in recent 6 months and HIV testing in recent 1 year were 35.5%（ 95%CI = 23.8%-47.2%） and 44.3%（ 95%CI = 34.0%-54.6%）. Conclusion The high prevalence of HIV infection and the low rates of condoms using and HIV testing among MSM students in Chinese mainland asked for great concern to HIV control and intervention.
