摘 要:目的探讨云南中小学生睡眠时间与中心性肥胖的关系,为制定相应的预防和干预策略提供科学依据。方法采用分层随机整群抽样方法,抽取云南省昆明市、大理州、红河州7~18岁汉族学生7 200名,进行身高、腰围测量和睡眠时间等调查。结果 7~18岁中小学生中心性肥胖检出率为21.7%,男生低于女生,初中生低于小学生和高中生,经济好片和中片低于差片(χ~2值分别为21.437,15.114,38.346,P值均<0.01)。单因素Logistic回归分析显示,性别、年龄、经济片区因素对儿童青少年中心性肥胖率的影响均具有统计学意义(P值均<0.05);进一步控制人口学因素、静态和动态活动因素后的Logistic回归分析显示,睡眠时间与9~<10 h相比,<6 h的中小学生发生中心性肥胖的相对危险度为1.567(95%CI=1.121~2.190),其余睡眠时间对中小学生中心性肥胖的影响均无统计学意义。结论睡眠时间不足是云南中小学生中心性肥胖的独立危险因素,应通过增加儿童青少年的睡眠时间来预防和控制中心性肥胖的流行。Objective To investigate the relationship between sleep duration and central obesity among primary and middle school in Yunnan for providing scientific evidence for making prevention and intervention strategies of central obesity. Methods A cross-sectional study was conducted among 7 200 Han students aged 7 to 18 selected by stratified random cluster sampling method in Kunming, Dali, Honghe. And their height, waist circumstance and sleep duration were measured. Results The detection rate of central obesity among children and adolescents aged 7 to 18 years was 21.7%, and the rate of boys was lower than that of girls, the junior school students got a higher rate than the elementary and high school students, the students in the first and second economic areas got a higher rate than those in the third economic area( χ^2= 21.437, 15.114, 38.346, P〈0.01). Univariate logistic regression analysis revealed that the impact of the various demographic factors on the rates of central obesity among primary and middle school students was statistically significant( P〈0.05); logistic regression analysis of further controlling the demographic, static and dynamic factors revealed that compared with the primary and middle school students with the sleep duration of 9-〈 10 h, those with sleep time less than 6 h had a relative risk of catching central obesity, which equaled 1.567( 95% CI = 1.121-2.190), the effect of other sleep duration on central obesity remained not statistically significant. Conclusion Short sleep is an independent risk factor of central obesity among primary and middle school students. The prevalence of central obesity should be prevented and controlled by increasing sleep time of children and adolescents.
