摘 要:目的观察chemerin在膝骨关节炎关节液和软骨中的表达,初步探讨其参与调节软骨代谢的机制,在膝骨关节炎发病中的作用。方法收集膝骨关节炎(观察组)患者和截肢或股骨颈骨折(对照组)关节软骨,同时收集膝骨关节炎关节液。采用ELISA检测关节液中chemerin浓度,实时荧光定量聚合酶链式反应(qRT-PCR)和蛋白质印迹法(Wb)检测软骨中chemerin/chemeR23及PPARγ2表达量。结果膝骨关节炎K-L 4级患者关节液中chemerin浓度比K-L 2、K-L 3级患者明显增高,分别为(13.37±2.01)和(11.24±1.78)、(11.61±1.98)μg/L,差异有统计学意义(P<0.001),关节液中chemerin浓度与膝骨关节炎K-L分级呈正相关关系(r=0.367,P<0.001);qRT-PCR结果显示观察组与对照组软骨中chemerin和PPARγ2mRNA表达量的比值即2^(-ΔΔCT)分别为0.428±0.286、0.658±0.173和0.261±0.182、0.599±0.350,观察组高于对照组,差异有统计学意义(P<0.05)。Wb结果显示观察组和对照组关节软骨chemerin和PPARγ2蛋白表达量分别为220.69±25.03、190.71±39.54和189.33±43.15、137.64±35.52,观察组高于对照组,差异有统计学意义(P<0.05)。相关分析显示chemerin和PPARγ2之间存在正相关关系(r=0.542,P<0.001)。结论关节液中chemerin浓度与膝骨关节炎严重程度呈正相关。chemerin和PPARγ2在骨关节炎软骨中呈高表达,且两者间存在正相关关系,可能参与软骨代谢的调节,促进骨关节炎发生发展。关节液中chemerin浓度是评价骨关节炎严重程度的新指标。Objective To study the roles of Chemerin in the pathogenesis of osteoarthritis (OA) via investigating the Chemerin expression in synovial fluid (SF) and cartilage. Methods Knee SF and cartilages of patients with knee OA were collected as experimental group, and normal cartilage of patients with femoral neck fracture or amputation were collected control group. Quantitative realtime polymerase chain reaction (qRT- PCR) and Western- blot (Wb) were used to assess the expression of Chemerin/chemeR23 and PPARγ2 in cartilage, as enzyme - linked immunosorbent assay (Elisa) was used to assess the Chemerin concentration in SF. Results The Chemerin concentration in SF of' patients with Kellgren - Lawrence 4 ( K - L4) OA was significantly higher than those of patients with K - L2 and K - L30A, and there was a positive liner correlation between Chemerin and K - L grading of OA. QRT - PCR and Wb results demonstrated that the expression of Chemerin and PPARγ2 in OA cartilage was significantly higher than that in control group, and there was a positive correlation between Chemerin and PPARγ2. Conclusion The concentration of Chemerin in SF increases with the degree of knee OA according to K - L grading. The expression levels of chemerin and PPARγ2 in OA cartilage are higher than the control, and there is a positive liner relationship between Chemerin and PPARγ2. Chemerin may play an important role in the occurrence and development of knee OA. The Chemerin concentration in SF may be a new index for the degree of OA.
