摘  要:目的了解四川省南充市耐药结核病耐药现状及特点,为制定耐药结核病控制策略提供参考依据。方法收集2014-08/2017-12南充市9县(市、区)提供的476例患者的痰培养标本,进行异烟肼(INH)、利福平(RFP)、乙胺丁醇(EMB)、链霉素(SM)、氧氟沙星(OFX)和卡那霉素(KM)6种传统结核病治疗药物的药敏检测,分析其耐药情况和流行特征。结果南充市总耐药率31.3%,耐多药率15.7%,广泛耐药率0.6%;复治患者耐多药率高于初治患者;耐药顺位是从高到低依次是INH、RFP、SM、OFX、EMB和KM;性别、城乡耐药差异无统计学意义,年龄组耐药高峰在40~59岁。结论南充市的耐药率高于2010年四川省第五次流行病学调查水平,耐药结核病控制形势较为严峻,耐药患者的发现、治疗管理是今后的重点工作。Objective To investigate current drug resistance of Tuberculosis(TB) in Nanchong and provide some ferences for the control of drug-resistant TB.Methods The sputum specimen of 476 cases from 9 counties(cities/districts) of Nanchong from Aug.2014 to Dec.2017 were collected.The 6 kinds of traditional tuberculosis drugs,in cluding Isoniazide(INH),rifampin(RFP),ethambutol(EMB),streptomycin(SM),ofloxacin(OFX) and kanamycin(KM),were conducted to analyze the drug resistance and epidemic characteristics.Results The total resistance rate was 31.3%,the multidrug resistance(MDR-TB) rate was15.7%,and the extensive drug-resistance(XDR-TB) rate was 0.6%.The MDR-TB rate of retreatment patients was higher than that of the initial treatment patients.The drug resistance rate of the 6 traditional tuberculosis drugs was INH,RFP,SM,OFX,EMB,KM.There was no significant difference between different gender and residence population,patients in 40-59 years old showed higher drug resistance than others.Conclusion The drug resistance rate in Nanchong is higher than that in the fifth epidemiological survey in Sichuan province issued in 2010,The situation of drug-resistant tuberculosis is serious,the diagnosis,identifying and treatment management of drug-resistant TB should be the key work in the future.
