摘 要:目的探讨无创与有创通气方式对慢性阻塞性肺疾病(COPD)并Ⅱ型呼吸衰竭急诊患者的疗效。方法将80例COPD并Ⅱ型呼吸衰竭患者根据随机数字表法分为有创组和无创组,每组40例。有创组患者采用有创呼吸机通气进行治疗,无创组患者通过无创呼吸机通气进行治疗,比较两组患者治疗24 h后血气指标及治疗结局的差异。结果两组患者治疗后二氧化碳分压、动脉血氧分压、动脉血氧饱和度均明显改善,有创组上述指标改善程度比无创组明显,差异均具有统计学意义(P<0.05);两组患者放弃率、好转率与死亡率组间比较均无显著性差异(P>0.05)。结论有创通气能迅速改善患者血气指标,且治疗结局与无创通气相似。Objective To evaluate the efficacy of noninvasive and invasive ventilation in patients with COPD and type Ⅱ respiratory failure. Methods 80 cases of COPD and type Ⅱ respiratory failure patients were divided into invasive group and non-invasive group according to the random number table method, 40 cases in each group. The invasive group was treated with invasive ventilator ventilation, and the non-invasive group was treated with noninvasive ventilator ventilation. The differences in blood gas parameters and treatment outcomes after 24 hours of treatment between the two groups were compared. Results The levels of PaCO2, PaO2 and SaO2 in two groups were significantly improved after treatment. The improvement of The improvement of the above indexes in invasive group was significantly higher than the noninvasive group(P 〈 0.05). There was no significant difference in the rate of abandonment, improvement and mortality between two groups(P 〉 0.05). Conclusion Invasive ventilation can rapidly improve the blood gas index of patients, the treatment outcome is similar to noninvasive ventilation.
