芪苈强心胶囊治疗慢性心力衰竭的临床疗效观察

摘　　要：目的探讨芪苈强心胶囊治疗慢性心力衰竭的临床疗效。方法选取2015年2月—2015年10月于天津市人民医院住院治疗的88例慢性心力衰竭病人,随机分为常规治疗组(西药组)和常规治疗协同芪苈强心胶囊治疗组(观察组),各44例。西药组给予限盐等一般治疗,根据病情给予利尿剂、肾素-血管紧张素转换酶抑制剂(ACEI)/血管紧张素Ⅱ受体拮抗剂(ARB)、醛固酮受体阻断剂、β受体阻滞剂等药物,观察组在西药组治疗基础上加用芪苈强心胶囊治疗,每次4粒口服,每日3次。收集入组病人一般临床资料及明尼苏达州心力衰竭质量问卷(MLHF)评分、中医证候积分、左室射血分数(LVEF)、左心房内径(LAD)、舒张期左心室内径(LVDd)及血清N端B型脑钠肽前体(NT-proBNP)等指标,治疗6个月后复测上述指标。结果治疗6个月后,观察组总有效率明显高于西药组(P<0.05)。两组病人治疗前MLHF评分、NT-proBNP、LVEF、LAD、LVDd、中医证候积分比较,差异无统计学意义(P>0.05);两组治疗后LAD、LVDd水平均较治疗前下降(P<0.05),LVEF较治疗前明显改善(P<0.01)。两组病人治疗后WLHF评分,中医证候积分及NT-proBNP均较治疗前下降(P<0.05),观察组下降更显著(P<0.05)。结论芪苈强心胶囊治疗慢性心力衰竭能显著改善病人临床症状,提高病人生活质量。Objective To explore the effects of Qili Qiangxin capsule（QQC）in the treatment of chronic heart failure（CHF）.Methods Eighty-eight patients with CHF hospitalized in Tianjin Union Medical Center from February 2015 to October 2015 were randomly divided into two groups：control group（n=44）treated with western medicine routine treatment,and treatment group（n =44）treated with QQC in the basis of western medicine routine treatment for 6 months.The clinical efficacy,Minnesota Living with Heart Failure Questionnaire（MLHF）scores,traditional Chinese medicine（TCM）symptom scores,left ventricular ejection fraction（LVEF）,left atrial diameter（LAD）,diastolic left ventricular diameter（LVDd）,and serum N-terminal pro-B-type natriuretic peptide（NT-proBNP）levels were observed before and after six months of treatment.Results There was no significant difference in MLHF scores,serum NT-proBNP,LVEF,LAD,LVDd,and TCM syndrome scores between the two groups before treatment（P〉0.05）.After 6 months of treatment,the total clinical effective rate was significantly higher in treatment group than that in control group（P〈0.05）.The scores of Minnesota quality of life and levels of serum NT-proBNP,LAD,and LVDd were decreased in treatment group compared with control group（P〈0.05）.LVEF was also dramatically improved in treatment group（P〈0.05）.Conclusion QQC can improve the clinical symptoms and enhance the quality of life in patients with CHF.
