经尿道纽扣式电极等离子汽化术治疗非肌层浸润性膀胱癌的疗效分析

摘　　要：目的:探索经尿道纽扣式电极等离子汽化术(TURis)治疗非肌层浸润性膀胱癌的疗效及其安全性。方法:收集2015年3月~2017年9月应用TURis及经尿道膀胱肿瘤电切术治疗膀胱肿瘤共56例,其中TURis组26例,双极电切组30例。收集病例的临床资料,术后3个月随访,行泌尿系超声检查和膀胱镜检查。结果:56例手术均成功,TURis组的术后平均膀胱冲洗时间及平均留置尿管时间分别为1.80±0.63d和3.19±1.38d,双极电切组的分别为2.93±1.51d和4.70±1.95d,2组间比较无显著性差异(P>0.05)。TURis组的术中平均出血量(8.23±8.84ml)显著低于双极电切组(13.90±10.17ml,P<0.05)。双极电切组有2例出现较长时间的轻度血尿。术后3个月,TURis组复发1例;双极电切组复发3例。结论:与双极电切术相比,TURis术后平均膀胱冲洗时间及平均留置尿管时间相对较短、出血量少,治疗非肌层浸润性膀胱癌安全有效。Objective：To investigate the efficacy and safety of transurethral resection in saline（TURis）button electrode plasma vaporization in the treatment of non-muscle-invasive bladder cancer（NMIBC）.Methods：From March 2015 to September 2017,26 cases with NMIBC were treated by plasma button electrode vaporization,30 cases were treated by conventional transurethral resection of the bladder tumor（TURBT）.Clinical data of the patients were collected,as well as the results of a 3-month follow-up including urinary tract ultrasounography and cystoscopy.Results：All the 56 surgeries were successful.The average postoperative bladder irrigation time and mean postoperative urinary catheterization time had no significant difference between the two groups（P〉0.05）.The average intraoperative blood loss in the button electrode plasma vaporization group（8.23±8.84 ml）was significantly less than that of the TURBT group（13.90 ±10.17 ml,P 〈0.05）.Except for two cases of mild hematuria that occurred for a long time in the TURBT group,no significant complications occurred in the other kind of operations.During the 3-month follow-up,in the button electrode group there was 1 case of recurrence in situ;in the TURBT group,there were 3 cases of recurrence.Conclusion：Compared with TURBT,the average bladder irrigation time and mean indwelling catheter time of TURis is relatively short.TURis is safe and effective,owning the advantage of reducing blood loss with more clarity in the treatment of NMIBC.
