腰椎Modic、Hiz现象与腰背疼痛之间的调查分析

摘　　要：目的:研究探讨无腰背疼痛人群中Hiz及Modic分布情况,以及与腰背疼痛之间的关系。方法:收集189名无腰背痛志愿者及200腰背痛患者行MRI检查,分别评估Hiz及Modic出现的年龄阶段、腰椎节段,以及不同类型分布特点,并分析二者之间的关联性。结果:志愿者中单有Hiz的有54例,单有Modic的有22例,而二者都共有的有23例,而对照组中分别为59、32、35例,Modic差异存在统计学意义(P<0.05),Hiz则无统计学意义。二者主要集中于50岁以上人群,L4/5及L5/S1节段多见(P<0.05)。志愿者以ModicⅡ型为主,而对照组中ModicⅠ型明显增多(P<0.05)。Hiz和Modic之间Spearman秩相关系数两人群分别为0.118、0.075,P>0.05。结论:Hiz及Modic不能单独作为腰背痛的评价指标,二者可相互独立存在,Modic与腰背疼痛有一定关联,而且以ModicⅠ型为著,Hiz则与腰背疼痛无明显关联。Objective：To investigate the distribution of Modic and Hiz in people with no lumbar back pain,and the relationship with back pain.Methods：189 volunteers with no lumbar back pain and 200 patients with lumbar back pain were collected,all of whom had MRI examination,and the age,lumbar segment,the different distribution characteristics of Hiz and Modic were evaluated respectively,and the relationship between them were analyzed.Results：There were 54 cases with Hiz,22 cases with Modic,and 23 cases with both,while there were 59 cases,32 cases,and 35 case in the control group respectively,the difference of Modic was statistically significant（P〈0.05）,but there was no significant difference in Hiz between the two groups（P〈0.05）.Hiz and Modic mainly concentrated in the population over 50 years old,and they were more common in L4/5 and L5/S1 segments（P 0.05）.Modic type Ⅱ was the main type in the volunteers,while Modic typeⅠincreased significantly in the control group（P〈0.05）.The Spearman rank correlation coefficient between Hiz and Modic was 0.118 and 0.075,respectively（P〉0.05）.Conclusion：Hiz and Modic can not be used as the criterion for the lower back pain,and each of them can exist independently.Modic has some correlation with lower back pain,and mainly with Modic type.Hiz is not associated with lower back pain.
