益肾活血通络方对膝骨关节炎患者血清脂联素及软骨寡聚基质蛋白的影响

摘　　要：目的探讨益肾活血通络方对膝骨关节炎(KOA)患者血清脂联素及软骨寡聚基质蛋白(COMP)的影响。方法选择医院骨科就诊的KOA患者165例,按照随机数字表法分为对照组(83例)和观察组(82例),对照组患者口服盐酸氨基葡萄糖胶囊治疗,观察组患者给予益肾活血通络方治疗,均连续治疗8周。结果观察组患者总有效率为89.02%,高于对照组的67.47%(P<0.01);观察组脂联素及COMP水平均低于对照组(P<0.01),血清炎性因子白细胞介素(IL)1、IL-6及肿瘤坏死因子-α(TNF-α)水平均低于对照组(P<0.01),膝关节评分(WOMAC)及全膝关节核磁共振成像积分(WORMS)均低于对照组(P<0.01);观察组患者不良反应发生率为7.32%(6/82),对照组为6.02%(5/83),组间比较无明显差异(P>0.05)。结论益肾活血通络方能提高膝关节炎的临床疗效,降低炎性因子水平,改善影像学损伤,保护膝骨关节。Objective To investigate the effect of Yishen Huoxue Tongluo Recipe on serum adiponectin and cartilage oligomeric matrix protein（ COMP） in patients with knee osteoarthritis. Methods Totally 165 patients with knee osteoarthritis in the Department of Orthopedic of our hospital were divided into the control group（ n = 83） and the observation group（ n = 82） according to the random number table method. The control group was treated with oral administration of Glucosamine Hydrochloride Capsules, while the observation group was treated with Yishen Huoxue Tongluo Recipe. The two groups were treated for 8 weeks continuously. Results The total effective rate of the observation group was 89. 02%, which was higher than 67. 47% of the control group（ P〈0. 01）. The levels of adiponectin and COMP in the observation group were lower than those in the control group（ P〈0. 01）. The levels of serum inflammatory cytokines（ IL-1,IL-6 and TNF-α） in the observation group were lower than those in the control group（ P〈0. 01）. The scores of WOMAC and WORMS in the observation group were lower than those in the control group（ P〈0. 01）. The incidence rate of adverse reactions in the observation group was 7. 32%（ 6/82）, which was not significantly different from 6. 02%（ 5/83） in the control group（ P〈0. 05）. Conclusion Yishen Huoxue Tongluo Recipe in the treatment of patients with knee osteoarthritis can improve the clinical effect,reduce the levels of inflammatory factors,improve the imaging damage and protect the knee joint.
