丹红注射液对急性脑梗死外周血中性粒细胞淋巴细胞比值的影响及其临床意义

摘　　要：目的探讨丹红注射液对急性脑梗死外周血中性粒细胞淋巴细胞比值(NLR)的影响及其与病情、预后的关系。方法选取医院2016年1月至2017年2月收治的急性脑梗死患者86例,在常规治疗基础上采用丹红注射液20 mg/d连续静脉注射,治疗14 d。结果与治疗前比较,患者治疗1周和治疗2周的外周血NLR、美国国立卫生研究所脑卒中评分(NIHSS)和急性生理学与慢性健康状况评分(APACHEⅡ)均降低(P<0.05);患者28 d病死率为9.30%(8/86);与存活患者比较,死亡患者治疗前后的外周血NLR,NIHSS评分和APACHEⅡ评分均降低(P<0.05);Pearson线性相关分析结果显示,丹红注射液治疗急性脑梗死外周血NLR与其NIHSS评分和APACHEⅡ评分均呈正相关(r=0.872,0.921,P<0.05);Spearman无条件相关分析结果显示,丹红注射液治疗急性脑梗死外周血NLR与其28 d病死率亦呈正相关(r=0.886,P<0.05)。结论丹红注射液有助于改善急性脑梗死外周血病情和神经损伤,且其病情、神经损伤及预后均与外周血NLR相关,外周血NLR可能作为其病情、神经损伤及预后评估的参考指标。Objective To investigate the effect of Danhong Injection on the peripheral blood neutrophil-lymphocyte ratio（ NLR） in patients with acute cerebral infarction（ ACI） and its relationship with the disease severity and prognosis. Methods Totally 86 patients with acute cerebral infarction admitted to our hospital from January 2016 to February 2017 were selected. On the basis of routine treatment, all the patients were given continuous intravenous injection of Danhong Injection 20 mg/d for 14 d. Results Compared with before treatment, the peripheral blood NLR,NIHSS score and APACHE Ⅱ score of the patients were decreased after 1,2 weeks of treatment（ P〈0. 05）. The 28 d mortality rate of the patients was 9. 30%（ 8/86）,compared with the survived patients,the peripheral blood NLR, NIHSS score and APACHE Ⅱ score of the death patients were decreased before and after treatment（ P〈0. 05）. Pearson linear correlation analysis results showed that Danhong Injection in the treatment of peripheral blood NLR in patients with acute cerebral infarction had positive correlation with the NIHSS score and APACHE Ⅱ score（ r = 0. 872,0. 921, P〈0. 05）. Spearman unconditional correlation analysis results showed that Danhong Injection in the treatment of peripheral blood NLR in patients with acute cerebral infarction had positive correlation with the 28 d mortality rate（ r = 0. 886, P〈0. 05）. Conclusion Danhong Injection can improve peripheral blood condition and nerve injury in patients with acute cerebral infarction, and the disease severity, nerve injury and prognosis are related to peripheral blood NLR. The peripheral blood NLR may be a reference index for evaluating the disease severity,nerve injury and prognosis of patients.
