养血清脑颗粒联合右佐匹隆片治疗急性脑卒中后失眠疗效观察

摘　　要：目的探讨养血清脑颗粒联合右佐匹克隆片治疗急性脑卒中后失眠的临床疗效。方法选取医院2014年至2015年收治的急性脑卒中后失眠患者118例,采用随机数字表法分为观察组60例和对照组58例。对照组患者给予右佐匹克隆片治疗,观察组患者在对照组治疗基础上加用养血清脑颗粒,共治疗4周。结果与对照组相比,观察组临床显效率为53.33%(32/60),总有效率为93.33%(56/60),均明显提高(P<0.05);观察组患者睡眠质量、入睡时间、睡眠时间、睡眠效率、睡眠障碍、催眠药物、日间功能评分和匹兹堡睡眠质量指数总分均明显降低(P<0.05);观察组患者血5-羟色胺含量为(129.75±23.86)ng/m L,血γ-氨基丁酸含量为(9.86±1.97)ng/L,均明显提高;血多巴胺含量为(76.38±8.74)ng/L,明显降低(P<0.05);观察组患者中国卒中量表评分为(14.13±0.75)分,明显降低,而蒙特利尔认知评估量表评分为(26.08±3.71)分,明显提高(P<0.05);观察组患者日常生活能力评分分别为(48.75±5.98)分和(68.96±7.91)分,均明显提高(P<0.05)。结论血清脑颗粒联合右佐匹克隆片治疗急性脑卒中后失眠,可改善患者失眠症状,促进神经功能恢复,提高日常生活能力,临床疗效显著。Objective To investigate the clinical effect of Yangxue Qingnao Granules combined with Eszopiclone Tablets in the treatment of insomnia patients after acute stroke. Methods Totally 118 insomnia patients after acute stroke admitted to our hospital from 2014 to2015 were selected and divided into the observation group（ n = 60） and the control group（ n = 58） by the random number table method. The control group was treated with Eszopiclone Tablets, on this basis, the observation group was treated with Yangxue Qingnao Granules. The two groups were treated for 4 weeks. Results Compared with the control group, the clinical treatment effective rate（ 53. 33%,32/60） and total effective rate（ 93. 33%,56/60） in observation group were significantly increased（ P〈0. 05）,the scores of sleep quality,fall time,sleep time,sleep efficiency,sleep disorder,hypnotic drug,daytime function and total scores of PSQI in observation group were significantly decreased（ P〈0. 05）. Compared with the control group, the levels of 5-HT [（ 129. 75 ± 23. 86） ng/m L ] and GABA [（ 9. 86 ± 1. 97） ng/L ] in the observation group were significantly increased,while the level of DA [（ 76. 38 ± 8. 74） ng/L ] in the observation group was significantly decreased（ P〈0. 05）, the CSS score [（ 14. 13 ± 0. 75） points ] in the observation group was significantly decreased,while Mo CA score [（ 26. 08 ± 3. 71） points ] in the observation group was significantly increased（ P〈0. 05）,the FMA score [（ 48. 75 ± 5. 98） points ] and BI score [（ 68. 96 ± 7. 91） points ] in the observation group were significantly increased（ P〈0. 05）.Conclusion Yangxue Qingnao Granules combined with Eszopiclone Tablets is effective in the treatment of insomnia patients after acute stroke,it can improve symptoms of insomnia,promote recovery of neurological function and improve ability of daily living.
