摘　　要：目的探讨不同病程痛风超声影像学特点及NACHT-LRR-PYD结构域蛋白3(NALP3)、环氧化酶-2(COX-2)、白细胞介素-1β(IL-1β)的水平。方法选择2016年10月至2017年10月收治的89例痛风患者进行前瞻性研究,根据不同病程,分为早期痛风组(A组,23例)及晚期痛风组(B组,66例)。观察对比两组的超声特征及NALP3、COX-2、IL-1β的水平。结果 B组双轨征、痛风石及骨侵蚀发生率显著高于A组,B组的NALP3、COX-2、IL-1β水平均明显高于A组(P<0.05)。结论病程≥3年者的双轨征、痛风石、骨侵蚀发生率及NALP3、COX-2及IL-1β水平显著高于<3年者,表明随着痛风病程的迁延,患者体内尿酸盐沉积负荷增加,而痛风疾病程度加重可能与NALP3、COX-2及IL-1β水平升高有关。Objective To investigate the relationship of NALP3,COX-2,IL-1β level and ultrasonic imaging features in patients withdifferent gout course. Methods 89 cases with different gout course were chosen from Oct 2016 to Oct 2017,who were divided into the early gout group（ A group,23 cases） and late gout group（ 66 cases）. The ultrasound features,NALP3,COX-2,IL-1β of two groups were observed and compared. Results The incidence rates of double track sign,gouty stone and bone erosion of B group were significantly higher than those of A group,the NALP3,COX-2,IL-1β level of B group were higher than A group（ P〈0. 05）. Conclusion The longer the duration of gout,the more severe the double track sign,gout stone,bone erosion and NALP3,COX-2 and IL-1β level,with the gout course increased,the deposition load of uric acid increases,the increase of gout disease degree may be related to the increase of NALP3,COX-2 and IL-1β level.
