无眩晕头晕症状良性阵发性位置性眩晕临床诊断探讨

摘　　要：目的探讨良性阵发性位置性眩晕(BPPV)诊断中是否存在无眩晕、头晕症状患者及其在BPPV患者中的比率。方法回顾性分析空军总医院眩晕诊疗中心2014年1月至2017年6月接诊的临床资料完整的278例与BPPV相关病例,其中221例以"眩晕、头晕"为主诉,57例以"头昏沉"而并无"眩晕、头晕"主诉患者经器械前庭变位试验检查和复位治疗被证实为与BPPV相关。结果日常状态下,"相对于重力方向改变头位后出现反复发作的、短暂眩晕或头晕"BPPV患者221例;日常状态下,相对于重力方向改变头位后无反复发作的、短暂眩晕或头晕,仅表现为"头昏沉感"BPPV相关患者57例。结论在BPPV相关诊断中,表现为"眩晕、头晕"症状者比率为79.5%(221/278),仅表现为"头昏沉感"患者占比20.5%(57/278)。Objective To investigate the presence and the ratio of patients without vertigo or dizziness in patients diagnosed with BPPV.Methods Clinical data of 278 benign paroxysmal positional vertigo（ BPPV） patients were collected from Jan 2014 to Jun 2017. 221 out of 278 patients complained of ＂ vertigo or dizziness＂,others just ＂ Giddy feeling＂. The choosing patients proved to be BPPV by instrumental Vestibular test and reduction therapy,were complained of ＂ vertigo or dizziness＂,but it＇s just ＂ Giddy feeling＂. Results After the change of gravity direction on head,there was 221 cases of BPPV who had recurrent,transient vertigo or dizziness,and 46 cases of BPPV presented just ＂ Giddy feeling＂.Conclusion Among the patients diagnosed with BPPV,the ratio of ＂ vertigo or dizziness＂ was 221/278（ 79. 5%）,and the proportion of just ＂Giddy feeling＂ was 57/278（ 20. 5%）.
