海洛因依赖患者美沙酮维持治疗前后心理健康状况变化及其与社会支持的相关性

摘　　要：目的观察海洛因依赖患者美沙酮治疗前及治疗1年后心理健康状况的变化情况及其与社会支持的相关性。方法204例海洛因依赖患者美沙酮维持治疗前及治疗1年后分别进行症状自评量表(SCL-90)评定。海洛因依赖患者在美沙酮维持治疗1年后同时进行社会支持评定量表(SSRS)评定。对海洛因依赖患者治疗前症状自评量表(SCL-90)与中国常模进行比较,并对海洛因依赖患者美沙酮维持治疗前及治疗1年后症状自评量表(SCL-90)进行比较。患者治疗前后症状自评量表(SCL-90)的减分率与社会支持评定(SSRS)的相关性采用Spearman相关分析。结果患者治疗前躯体化、人际敏感、抑郁、焦虑、敌对、偏执因子分高于常模,差异有统计学意义(P<0.05)。患者治疗1年后躯体化、人际敏感、抑郁、焦虑、敌对、偏执因子分较治疗前减少,差异有统计学意义(P<0.05)。患者人际敏感因子、抑郁因子、焦虑因子、敌对因子、偏执因子减分率与社会支持各维度评分均呈正相关(P<0.05)。结论海洛因会对人体同时产生躯体性症状和精神性症状。美沙酮维持治疗不仅改善患者的躯体性症状,同时也能改善患者的精神性症状。美沙酮维持治疗同时加强社会支持力度能够促进患者身心健康及重塑个人幸福感。Objective To explore the changes of psychological health and its relationship with social support before and after methadone maintenance therapy for one year in heroin-addicted patients. Methods A total of 204 cases of heroin addicts were evaluated by SCL-90 before and after methadone maintenance therapy for one year. Social support scales were evaluated after methadone maintenance therapy for one year. Results Before treatment,the scores of somatization,interpersonal sensitivity,depression,anxiety,hostility and stubborn in patients were significantly higher than those of the norm（ P〈0. 05）,After treatment,the scores of somatization,interpersonal sensitivity,depression,anxiety,hostility and stubborn decreased（ P〈0. 05）. Social support was positively related to the reduction rate of interpersonal sensitivity,depression,anxiety,hostility and stubborn（ P〈0. 05）. Conclusion Heroine can cause physical symptoms and mental symptoms,and methadone maintenance therapy can alleviate both the physical symptoms and mental symptoms. Combination of methadone maintenance therapy and social support enhancement can promote the physical and mental health of patients,and rebuild their personal happiness.
