肛提肌以下肛管直肠撕裂伤27例诊疗体会

摘　　要：为探讨肛提肌以下肛管直肠撕裂伤的诊断和治疗方法,回顾性分析2012—2017年收治的肛提肌以下肛管直肠撕裂伤27例,全部行急诊手术治疗。结果:11例行单纯修补缝合术,7例行局部修补并旁置负压引流,5例去除异物后一期缝合并置引流,4例重度撕裂伤行骶前引流并直肠肛管冲洗辅助治疗。本组全部治愈,无死亡病例。结果表明,对于肛提肌以下肛管直肠撕裂伤以修补加引流为主,彻底有效止血、保护肛管直肠环功能、早期使用足量广谱抗生素是手术成功的关键。This study was to explore the diagnosis and treatment methods for laceration in ano-rectum be- low levator ani,retrospectively analysed the data of 27 patients with this laceration who admitted and re ceived emergency surgery in author＇s hospital from 2012 to 2017 yr. As results,ll cases received alone su ture repair,7 received local suture repair associated with outside exclusive vacuum drainage,5 received pri- mary suture associated exclusive drainage after removing foreign body, and 4 received sacroanterior drain- age and assisted treatment with irrigation of ano-rectum due to their severe laceration;all the patients were cured,no death case. Results show that for above laceration it should be mainly treating with repair plus drainage,and throughly effectively stopping bleeding,protecting anal rectal ring＇s function, as well as early using enough broad-spectrum antibiotics which all are the key to successful surgery.
