实时三维超声心动图评估慢性肾脏病患者左室收缩功能的应用价值

摘　　要：目的通过实时三维超声心动图(RT-3DE)评估慢性肾脏病患者的左室收缩功能。方法对比慢性肾脏病患者和正常人的左室射血分数,左室容积及收缩同步性,分析慢性肾脏病患者左心室收缩功能障碍的变化趋势。结果 CKD1-3期组左室射血分数与正常组差异无统计学意义,CKD4-5期组左室射血分数降低。CKD1-3期组左室收缩期末容积(BSA校正后)增高,差异有统计学意义,CKD4-5期组左室舒张、收缩期末容积(BSA校正后)显著增高。CKD各组左室收缩同步性指标与正常组差异均有统计学意义。结论慢性肾脏病患者左室容积变化及收缩同步性异常可以反映出左室收缩功能障碍。实时三维超声心动图用于评估CKD患者左室收缩功能具有良好的应用价值。Objective Assess the left ventricular systolic function in chronic kidney disease patients using real-time three-dimensional echocardiography. Methods The left ventricular ejection fraction,left ventricular volumes,left ventricular systolic synchronicity were measured in chronic kidney disease patients and compared to normal control group. Analyzing the change trend of left ventricular systolic function of patients suffered from chronic kidney disease. Results There was no significant difference of LV-EF between the CKD group（ grade 1 to 3） and normal control group. However,the LV-EF of CKD（ grade 4 to 5） decreased compared to normal control group. The end-stystolic volume（ normalized by BSA） of CDK1-3 group was higher than normal control group. The end-stystolic and end-diastolicvolume（ normalized by BSA） was significantly increased than normal control group. The left ventricular systolic synchronicity was significantly different among the CKD groups and normal control group.Conclusions Both the change of left ventricular volume and systolic synchronicity can reflect the obstacle of left ventricular systolic function in patients with chronic kidney disease.RT-3 D echocardiography has high value in estimating the left ventricular stystolic function.
