结肠癌根治术前应用CT血管造影的意义

摘　　要：目的研究结肠癌根治术前应用CT血管造影(CTangiography,CTA)进行血管评估的意义。方法选取2015年4月—2017年3月在蚌埠医学院第一附属医院就诊并行腹腔镜辅助结肠癌根治术的患者104例,按术前检查方式的不同分为CTA组64例与非CTA组(未行CTA检查)40例。术前所有患者均行全腹部CT检查,CTA组利用立体渲染技术显示肠系膜主要血管。手术指标比较:手术时间、术中出血量、淋巴结检出个数;术后并发症比较:切口感染、吻合口瘘、术后肠梗阻。结果 CTA组手术时间较非CTA组显著缩短、术中出血量较CT组显著减少,差异均有统计学意义(P<0.05)。2组术中淋巴结检出个数无统计学意义(P>0.05)。两组术后并发症发生情况差异无统计学意义(P>0.05)。结论通过术前CTA检查,了解肠系膜血管走形、变异及淋巴结分布情况,有助于缩短腹腔镜结肠癌手术时间、减少术中出血、降低术后并发症的发生。Objective To research the application value of CT angiography（ CTA） in preoperative assessment of colon cancer radical correction. Methods 104 colon cancer patients were chose,the patients received laparoscopic assisted colon cancer radical correction in the first affiliated hospital of Bengbu Medical University during April 2015 and March 2017. According to the methods of preoperative examination,all the patients were divided into 2 groups. CTA group： 64 patients with the CTA and CT examination,Non-CTA group：40 patients with CT examination only. The CTA group patients were combined with stereoscopic rendering technique to show major mesenteric vessels. The following surgical indicators were compared： operation time,intra-operative blood loss, lymph node number captured and postoperative complications（ incision infection,anastomotic fistula and postoperative intestinal obstruction）.Results Operation time was significantly abbreviated in CTA group than in non-CTA group（ P〈0. 05）. The intraoperative blood loss of CTA group was simply lower than that in non-CTA group（ P〈0. 05）. There were no special differences in lymph node number and post operative complications between CTA group and non-CTA group（ P〉0. 05）. Conclusions Through CTA examination,understand the mesenteric vessel style,variation and distribution of lymph nodes before colon cancer radical correction,helps to shorten the laparoscopic colon surgery time,reduce intraoperative bleeding,reduce postoperative complications.
