摘  要:目的在寒区战伤救治中心静脉穿刺困难的条件下,探索快速有效的骨髓穿刺急救输液途径,建立骨髓输液动物模型并进行评价。方法选取20只健康的成年雄性比格犬,随机分成两组,每组各10只。A组为前肢外侧静脉穿刺输液组,行常规静脉穿刺输液;B组为跟骨穿刺输液组,利用自行研发的骨髓穿刺输液器进行输液。比较两组穿刺用时、单位时间输液量、血流动力学指标、药物代谢学数据及行为学评价指标。结果 B组平均穿刺用时为(26±7)s,显著高于A组的(15±5)s,两组间比较,差异有统计学意义(P<0.05),且通过反复练习后,B组穿刺用时明显缩短。正常压力条件下,两组输液量比较,差异无统计学意义(P>0.05);最大压力状态下,B组输液量显著高于A组,两组比较,差异有统计学意义(P<0.05)。两组血流动力学指标和药物代谢学数据比较,差异无统计学意义(P>0.05)。B组实验犬出现轻微跛行和舔舐伤口,A组无明显异常行为。结论在寒区战伤急救环境下,跟骨骨髓输液途径是一种快速、有效的静脉输液替代途径。Objective To investigate a rapid and effective alternative method of emergency infusion in the condition of the central venous puncture failure,and to establish and evaluate animal model of intraosseous infusion. Methods A total of healthy adult male beagles were randomly divided into Group A and Group B,with 10 in each group. Beagles in Group A were consumed in regular venipuncture way,and the ones in Group B were consumed in intraosseous infusion way by own-make apparatus. The duration of puncture,amount of infusion,hemodynamic analysis,drug metabolism analysis and behavior evaluation between the two groups were compared.Results The duration of puncture in Group B was( 26 ± 7) seconds,which was longer than that in Group A( 15 ± 5) seconds( P〈0. 05); after practicing,the duration of puncture could be shorten significantly. Under the normal pressure,the infusion volume had no statistically significant difference between the two groups( P〈0. 05); under the max pressure,the infusion volume of Group B was significantly larger than that of Group A( P〈0. 05). There was no statistically significant difference of the hemodynamic analysis and drug metabolism analysis between the two groups( P〉0. 05). Beagles in Group B showed slight claudication and licking wounds,the ones in Group A had no obvious abnormal behavior. Conclusion The intraosseous infusion is a rapid and effective alternative emergency method of trauma and war wound.
